摘要
目的对动脉溶栓联合血管成形术治疗急性脑动脉闭塞的疗效进行探讨。方法将42例急性脑动脉闭塞患者随机分为2组,对照组21例,即对急性脑动脉闭塞的患者行rt-pt动脉溶栓后未联合血管成形术治疗;实验组21例,即对急性脑动脉闭塞的患者rt-pt动脉溶栓后联合血管成形术治疗;分别于溶栓后1d,7d,14d,1个月,3个月,6个月12个月随访观察溶栓效果及血管再发闭塞情况。结果对照组21例患者中,溶栓后立即行DSA造影,示血管完全再通10例,部分再通9例,2例无再通,术后6个月中随访观察显示7例患者出现血管再发闭塞,并2例因再栓塞死亡。实验组21例患者中,溶栓后示血管完全再通8例,部分再通12例,1例无再通,与对照组之间比较无统计学意义(P>0.05),术后6个月中示2例患者出现了血管再发闭塞,无死亡病例,与对照组之间比较有统计学意义(P<0.05)。结论动脉溶栓联合血管成形术治疗治疗急性脑动脉闭塞可以有效的降低血管再闭塞的发生,提高动脉溶栓的最终疗效。
Objective discuss the efficacy of artery thrombolysis combined with angioplasty of Acute cerebral artery occlusion. Method 42 cases of Acute cerebral artery occlusion were randomly divided into two groups, control group 21 cases, Acute cerebral artery occlusion patients with rt-pt arterial thrombolysis is not a joint angioplasty; experiment group 21 cases, joint angioplasty in patients with Acute cerebral artery occlusion after rt-pt intra-arterial thrombolysis; after thrombolysis, ld, 7d, 14d, 1 month, 3 month, 6 month 12 month follow-up observation thrombolytic effect and vascular re-issued occlusion. Results in 21 patients of the control group immediately after thrombolysis with DSA angiography shows vascular complete recanalization of 10 cases, part of recanalization of 9 cases, no recanalization of 2 cases, after 6 months follow-up study showed 7 cases of patients with vascular recurrent occlusion, and 2 cases of death re-embolization. 21 patients in the experimental group, after thrombolysis shows vascular complete recanalization of 8 cases, part of recanalization of 12 cases, patients without recanalization, no significant difference between the control group （P〉0.05）.after 6 months show that the 2 cases of patients with recurrent occlusion, with no deaths, statistically significant between the control group （P〈0.05）.Conelusion Angioplasty combined with artery thrombolytie therapy of Acute cerebral artery occlusion can effectively reduce the occurrence of vascular occlusion, improve the final outcome of artery thrombolysis.
