摘要
目的探讨异位妊娠超声图像的图像特点及阴道超声联合腹部超声诊断异位妊娠的优势。方法回顾性分析83例经阴道超声和腹部超声联合诊断为异位妊娠的病例,然后再经手术病理进一步证实超声的诊断符合率。结果在阴道超声和腹部超声联合诊断为异位妊娠的83例病例中,其中78例经手术证实异位妊娠,联合超声诊断的诊断符合率为94.0%,单纯腹部超声诊断异位妊娠69例,诊断符合率为83.1%。其中5例误诊,误诊率为6.0%。阴道超声联合腹部超声与单纯腹部超声比较,二者的诊断符合率有统计学差异(P<0.05)。异位妊娠图像特点:第一阶段单纯盆腔积液型5例,第二阶段孕囊型20例,第三阶段混合包块型58例。结论阴道超声联合腹部超声分阶段分析超声图像简便易行,值得推广。
Objective To study the characteristics of ultrasound sonogram of ectopic pregnancy and the superiority of com bined use of transvginal and trangabdominal ultrasound diagnosis. Methods All of 83 patients diagnosed as ectopic preg nancy were analyzed retrospectively,each patient of them received transvaginal ultrasound and transabdominal ultrasound before operation, and the results were further confirmed by pathological examination. Results Among the 83 patients diag nosed as ectopic pregnancy using transvaginal ultrasound and transabdominal ultrasound, 78 cases were proved to be ec topic pregnancy by operation and pathological examination, whose ultrasonic conforming rate was 94.0%. While only 69 eases were diagnosed as ecpotic pregnancy through transabdominal ultrasound,whose ultrasonic conforming rate was 83.1%.5 cases of them were misdiagnosed （6.0% ,5/83）. Coincidental rates of combined use of transvginal ultrasound and transabdominal ultrasound （94.0%） was significantly higher than that of transabdominal ultrasound alone,there was signifi cant difference between them （P 〈 0.05 ）. The characteristic of ectopic pregnancy sonogram were described as below ：there are 5 cases of single pelvic cavity liquid in the first stage, 20 cases of gestational sac type in the second stage, and 58 cases of mixing mass in the third stage. Conclusion The combined use of by transvginal ultrasound and transabdominal uhra sound and analysis of ectopic pregnancy using dividing stage sonogram was favorite choice in diagnose of ectopic pregnan cy.
