环孢素预处理对同种带瓣主动脉移植的保护作用

Protective effect of cyclosporine pretreatment on aortic valved homograft tissue

摘要 目的观察环孢素预处理在同种带瓣管道移植的作用。方法将1个月龄的中国白兔50只(取其带瓣主动脉)作为供体,体质量0.25~0.35kg;将成年中国白兔50只,体质量2.50~3.50kg,随机分2组作为受体,25只(移植时供体带瓣主动脉接受环孢素预处理)为实验组,25只(移植时供体带瓣主动脉不接受环孢素预处理)为对照组,移植前后应用葡萄糖代谢法检测带瓣血管的代谢,分别于第2、4周取出移植主动脉,观察血管是否通畅,苏木素.伊红(HE)染色组织病理图片观察,普通聚合酶链反应(PCR)鉴定MGP基因,荧光定量PCR2-△△Cr方法检测实验组4周、8周时血管平滑肌细胞中的MGPmRNA的相对表达量。结果(1)实验组通畅率为86.96%,明显高于对照组通畅率(59.09%),两组均数比较差异有统计学意义(P〈0.05);实验组(25例)葡萄糖代谢率均数4-标准差值为(4.3950±O.5642)mmoL/(L·24h),对照组(25例)葡萄糖代谢为(4.4720±0.7213)mmol/(L·24h),两组均数比较差异无统计学意义(P〉0.05)。(2)组织病理学观察,对照组明显出现内膜增生、中膜坏死和外膜浸润,随时间推移细胞成分明显减少,瓣膜正常分层状结构和结缔组织不断丧失;(3)MGP基因在实验组中表达,而在对照组中不表达;实验组中术后4周的MGPmRNA表达量(14.564±0.243)明显高于术后8周的表达量(6.492±0.962)。结论环孢素预处理的同种带瓣主动脉可提高移植血管通畅率,降低血栓形成,血管内皮细胞改变较轻;MGPmRNA水平的动态变化可能与环孢素抑制血管平滑肌细胞的表型转化及增殖有关。 Objective To observe the role of transplantation in the aortic valved homograft tissue with eyclosporine pretreatment. Methods White rabbits made in China only with 1 months as the donors (taking the aortas valved) were 50 cases, whose weight were 0. 25-0. 35 kg, white rabbit made in China whieh will be adult were 50 cases, whose weight were 2. 50-3.50 kg, divided into two groups as receptors randomly,among of them,25 eases were for the experimental group (donator with aortic valve accepted the pretreatment of cyclosporine when transplantation), and 25 cases ( donat with aortic valved unaccepted the pretreatment of cyclosporine when transplantation) were as the control group, adopting the method of glu cose metabolism to detect the metabolism of aortm valve before and after transplantation, then taking the aortic valve transplantation after 2,4 weeks, observed the open situation of artery and histopathological pic- ture by the colouration of hematoxylin and eosin ( HE), polymerase chain reaction (PCR) method verified MGP gene, Real-time PCR 2- A method detected the MGP expression in vessels of smooth muscle cells. Results ( 1 ) The vascular pateney rate was 86. 96% , which was higher than the control group (59.09%) significantly,there was statistically significant differences between them (P 〈 0. 05 ) ;the value of Glucose metabolic rate in the experimental group (25 cases) was (4. 3950±0. 5642) mmoL/( L-24 h) ,while that of the control group (25 cases) was (4. 4720 ± 0. 7213 ) mmol/( L.24 b), there was no statistically signifi- cant differences ( P 〉 0. 05 ). (2) During the histopatbologieal observation, the control group evidently dis- played neointimal hyperplasia, membrane necrosis and the outer membrane infiltrates, and cell components significantly reduced, the normal layered shape structure and normal connective tissue of valve would be continuous loss as time elapsed ; (3) MGP gene expression in the experimental group but not in the controls group;In the experimental group, the MGP mRNA expression content after 4 weeks postoperation ( 14. 564 ± 0. 243 ) was significantly higher than that of eight weeks ( 6. 492 ± 0. 962). Conclusion ( 1 ) Cyclosporine pretreatment on aortic valved homograft tissue can improve vessel patency rate after transplantation,reduce thrombosis,light the change of vascular endothelial cells; (2) MGP mRNA expression level of dynamic change may be connected with that cyclosporine inhibit the proliferation of transformation and the phenotype in vascular smooth muscle cell.
