摘要
目的探讨肺癌患者高凝状态的临床和实验室特点。方法对121例肺癌患者疾病临床特征及高凝状态指标进行统计分析与研究。结果 121例患者中有106例出现不同程度的凝血指标异常,7例发生静脉血栓栓塞症;非小细胞肺癌(NSCLC)中Ⅲ、Ⅳ期比Ⅰ～Ⅱ期患者的Fib、D-二聚体(DD)水平升高;NSCLC患者中腺癌比鳞癌及其他病理类型的DD水平升高;小细胞肺癌(SCLC)广泛期比局限期患者DD水平升高;体力状况评分(ECOG评分)2<ECOG<5组比ECOG≤2组Fib、DD水平高;合并疾病组比无合并疾病组DD水平高,以上各组间各项指标比较,差异均有统计学意义(P<0.05)。中心静脉置管组与无中心静脉置管组高凝状态相关指标之间比较,差异无统计学意义(P>0.05)。结论晚期肺癌患者、病理类型为腺癌及伴有合并疾病的肺癌患者Fib、DD水平升高,更易出现高凝状态;应早期发现并干预治疗,延长患者生存期,提高生活质量。
Objective To understand the clinical and laboratory characteristics of lung cancer with hypercoagulability. Methods The clinical data and hypercoagulabe indicators of 121 cases of lung cancer were statistically analyzed and studied. Results Among the 121 cases, hypercoagulabe indicators were abnormal in 106 cases,venous thromboembolism happened in 7 cases. For NSCLC, the mean D-dimer and FIB were significantly different between Ⅲ, Ⅳ stage and Ⅰ - Ⅱ stage;the mean D-dimer of adenocarcinoma was significandy higher than squamous cell carcinoma and other pathological category. For SCLC, the mean D-dimer of extensive stage was significantly higher than the limited stage. The mean D-dimer and the mean FIB of 2 〈 ECOG 〈 5 group were significantly higher than ECOG ≤ 2 group ; the mean D-dimer of the merger disease group was significantly higher than the single lung cancer group. All of the above mentioned differences were of statistical significance（ P 〈 0.05 ）. There were insignificant difference between the group of CVCS and the group of no CVCS （ P 〉 0.05 ）. Conclusion Advanced cancer patients of adenocarcinoma or with merger disease of lung cancer, often with higher FIB, DD level, are prone to complicate hypercoagulability, which should be detected early and interfered to prolong the survival and improve life quality.
