摘要
目的:探讨悬吊式免气腹腹腔镜下子宫肌瘤剔除术,经阴道子宫肌瘤剔除术及开腹子宫肌瘤剔除术的临床效果,并对此三种手术治疗方法的结果进行分析。方法:收集经阴道、免气腹腹腔镜下及开腹子宫肌瘤剔除术患者的资料,其中经阴道组45例,免气腹腹腔镜组40例,开腹组40例。记录三组的手术时间、术中出血量、应用抗生素时间、术后肛门排气时间、术后住院时间。结果:开腹组手术时间短于免气腹组和经阴道组,但术后首次排气时间、术后住院天数明显长于免气腹组及经阴道组,差异有统计学意义(P<0.05)。结论:经阴道子宫肌瘤剔除术具有术中出血量少、术后恢复早、住院时间短等优点。而免气腹腹腔镜手术,具有安全、经济、操作简便、手术适应证宽等优点。此两种新兴的手术治疗方法具有广阔的发展前景,值得临床借鉴。
Objective：To investigate the suspended gas-free laparoscopic uterine fibroids removed surgery,removed uterine fibroids by vaginal surgery and open surgery of uterine fibroids removed the clinical effect of the surgery,and this results of three surgical treatments for analysis.Methods：Transvaginal,gas-free laparoscopic and abdominal hysterectomy myomectomy surgery patient information,including transvaginal group of 45 cases,40 cases of gasless laparoscopy group,the laparotomy group（n=40）.Records of three groups of surgical time,blood loss,use of antibiotics,postoperative flatus and postoperative hospital stay.Results：Open operative time was shorter than the gasless group and transvaginal group,but after the first passage of flatus,and postoperative hospital stay was significantly longer than the free pneumoperitoneum group and transvaginal group,the difference was statistically significant（P〈0.05）.Conclusion：Transvaginal uterine fibroids removed surgery with less blood loss,postoperative recovery earlier,shorter hospital stay.Gasless laparoscopic surgery has the advantages of safe,economical and easy to operate,indications for surgery and wide.The two new surgical treatment method has broad prospects for development,worthy to learn from.
