摘要
目的:探讨分析舒张性心衰(diastolic heart failure,DHF)患者二尖瓣舒张早期血流峰速度/二尖瓣环舒张早期运动峰速度(early filling/early diastolicmitral annular velocity,E/Ea)与脑型钠利尿肽(brain natriuretic peptide,BNP)之间的相关性。方法:将我院心内科收治的72例DHF患者,按NYHA分级分为NYHAⅡ组、Ⅲ组和Ⅳ组,选择同期体检健康者40例作为健康组,分别对4组进行超声心动图检查测定E/Ea值,并测定血浆BNP质量浓度,分析两者的相关性。结果:DHF组E/Ea值和BNP质量浓度显著高于健康组(P<0.05),且随心衰程度的加重而升高。相关性分析表明E/Ea值和BNP质量浓度有高度相关(r=0.62,P<0.05),且随BNP质量浓度的升高而升高。结论:检测DHF患者的E/Ea能够很好地早期诊断DHF和评价左室舒张功能。
Objective: To evaluate the correlation between the ratio of mitral peak velocity of early filling(E) to early diastolicmitral annular velocity(Ea) and brain natriuretic peptide(BNP) in patients with diastolic heart failure. Methods: Mitral peak velocity of early filling was detected by Doppler tissue imaging and early diastolic mitral annular velocity by Doppler flow imaging. At the same time,measure the plasma BNP values. Results: E/Ea ratios and BNP values in patients with diastolic heart failure were dramatically higher than those in the controls (P〈0.05). E/Ea ratios and BNP values increased as the severity of heart failure. There had significant correlation between E/Ea ratios and BNP values (r=0.62, P〈0.05), E/Ea ratios increased with the increase of BNP values. Conclusion: E/Ea can well diagnosis diastolic heart failure early and evaluate the diastolic function of the left ventricle.
