摘要
目的探讨超声乳化白内障联合房角分离治疗原发性闭角型青光眼的临床疗效。方法根据房角粘连情况将患眼分为A组(房角粘连关闭≤180°)72眼、B组(180°<房角粘连关闭≤270°)106眼、C组(房角粘连关闭>270°)60眼,均行超声乳化白内障联合房角分离术。术前及术后2年随访视力、眼压、裂隙灯、眼底、房角、视野并进行统计学分析。结果 238眼中仅4眼视力无提高,无1例视力下降。3组术后眼压均较术前明显下降(P<0.01),房角均不同程度开放。A、B组术后视野平均光敏度(mean sensitivity,MS)较术前显著性增高(P<0.01),视野平均缺损(mean deviation,MD)较术前下降,差异有统计学意义(P<0.05)。C组术后MS较术前有下降趋势,MD较术前提高,但差异均无统计学意义(P>0.05)。结论对于原发性闭角型青光眼患者行超声乳化自内障联合房角分离术,能有效控制眼压,提高视力,保护视功能,手术并发症少,可作为原发性闭角型青光眼合并白内障患者安全有效的治疗选择。
Objective To explore the effectiveness of phacoemulsification combined with goniosynechialysis for primary angle - closure glaucoma（PACG）. Methods According to the situation of angle adhesion, the eyes were divided into group A （angle adhesion closed °〈 180°） of 72 eyes, group B （180°〈 angle adhesion closed °〈270°） of 106 eyes and group C（ angle adhesion close 〉 270°） of 60 eyes. All the patients were treated with phacoemulsification combined with goniosynechialysis. Visual acuity, intraocular pressure （IOP）, slit lamp, retina, visual field, were oberseved before and after operation. Results Visual acuity in all cases were raised expect four eyes which visual acuity were not reduced after phacoemulsification combined with goniosynechialysis. IOP was significantly lowered after operation（ P 〈 0.01 ）. Angle was open to varying degrees in 238 eyes after goniosynechialysis. The postoperative mean sensitivity （MS） of group A and group B were significantly increased compared with preoperation （P 〈 0.01 ）, mean deviation（MD） were absolutely decreased compared with preoperation, there were statistical differences （ P 〈 0.05 ）. MS had a downward trend and MD increased compared with the preoperation in group C, but had no statistical significance （P〉0.05）. ConclusionPhacoemulsification combined with goniosynechialysis is safe and effective to reduce IOP, complications and improves visual acuity in patients with PACG.
