某院433例Ⅰ类切口手术围术期抗菌药物应用合理性分析 被引量:3

摘要 目的了解并评价某院Ⅰ类切口手术围术期预防用药情况。方法随机抽取该院2010年1月-2012年2月出院的433份Ⅰ类切口手术病历进行调查及分析。结果该院Ⅰ类切口手术围术期预防用药比例达93.07%;282例(65.13%)首剂预防给药时间在术前0.5~2h;术后用药时间≤48h88例(20.32%);315例术后用药时间>48h,其中3例为治疗性用药,其余312例为预防性用药;预防用药选用药物品种繁多,共9类药物30个品种,应用最多的前3位分别是头孢唑啉(68例)、头孢地嗪(33例)和哌拉西林/舒巴坦(30例);单一用药比例为90.07%(390/433)。结论该院Ⅰ类切口手术围术期用药在把握用药指征、用药时机、合理选择药物及术后用药时间等方面都存在不同程度的不合理现象,仍需进一步规范。 Objective To find out and evaluate the reasonable analysis of perioperative application of antibacterial drug in 433 cases of type I incision operation. Methods Analyzed 433 cases medical records of type I incision operation during January in 2010 to February in 2012. Results The perioperative prophylactic application rate of antibacterial drug in type I incision operation reached 93.07% ;282 cases(65.13% ) received the first utilization of antibacterial drugs 0.5 -2h before operation ;88 cases (20.32%)were given antibacterial drug within 48h and 315 cases were given more than 48h after operation;During the 315 cases ,3 cases were therapeutic application of antibacterial drug, and the others were prophylactic. 9 catego- ries and 30 kinds of antibacterial drugs were used in perioperative period ; Cefathiamidine ( 68 cases ) , cefodizime ( 33 cases ) , piperacillin and sulbactam(30 cases) were the top 3 places in the list of frequency of antibacterial drugs use. The single medi- cation rate was 90.07% (390/433). Conclusion Perioperative application of antibacterial drug in the hospital is unreasonable in respect of irrational utilization, prophylactic medication time, choice of drug and long administration.
