摘要
目的探索社区卫生服务中心口腔医师在学校现场对一年级学生筛选后的六龄牙采用窝沟封闭适宜技术预防窝沟龋的发病,从而降低我区学生六龄牙窝沟龋的发病率。方法选择全区8所学校2 126名学生(年龄段为6~7岁的一年级学生),随机分为实验组:4所学校1 042名3 883颗六龄牙;对照组:4所学校1 084名4 141颗六龄牙。严格培训社区口腔医师,实验组经筛选后运用3M窝沟封闭剂在学校现场对1 672颗六龄牙进行窝沟封闭;对照组不予干预。向全区推广,并由第三方对封闭质量进行评估。结果社区口腔医师在学校现场进行窝沟封闭时间周期明显较门诊短;3年后六龄牙窝沟封闭剂脱落率15.47%;实验组较对照组窝沟龋发病率降低,有效率为80.9%;3年后在15个社区推广应用六龄牙窝沟封闭技术半年平均封闭剂脱落率为8.87%。结论社区卫生服务中心应用该技术的操作模式已在松江区各镇街道运用,其预防窝沟龋科学、合理、符合实际,效果肯定。
Objectives To study application of pit and fissure sealing in prevention of six-year-teeth dental caries screened from primary school students in communities.Methods A total of 2 126 primary students aged 6~7 from 8 schools were randomly divided into groups experiment(1 042 students from 4 schools,3 883 six-year teeth) and control(1 084 student from 4 schools,4 141 six-year teeth).A total of 1 672 six-year teeth of experiment group were given pit and fissure sealing using 3M pit and fissure seal in the spot,control group given no intervention.The sealing quality was evaluated by third party.Results Fissure sealing took less time in schools than in clinics.The rate of sealant loss rate was 15.47% after 3 years.The incidence of pit and fissure caries of experiment group was lower than that of control group,with an effective rate of 80.9%.This technique was generalized in 15 communities of the district after 3 years,its average half-year expulsion rate was 8.87%.Conclusion This operational model,having been used to prevent pit and fissure caries in towns and streets of Songjiang District,is scientific,reasonable,consistent with practical situation and effective.
