血清CA125与EmAb水平对子宫内膜异位症诊断价值的评价 被引量:2

摘要 为评价血清癌抗原 12 5(CA12 5)及抗子宫内膜抗体 (EmAb)水平对子宫内膜异位症 (内异症 )的诊断价值 ,以 87例需行腹腔镜或盆腔手术的住院患者为研究对象 ,其中内异症患者 (病例组 ) 71例 ,盆腔炎性包块患者 16例 (对照组 )。术前测定血清CA12 5及EmAb ,根据ROC方法确定CA12 5的最佳临界点。结果 :两组CA12 5及EmAb比较差别均无显著意义 (P >0 .0 5) ;CA12 5水平与美国生育协会的AFS分期无显著相关关系 (P >0 .0 5)。以 12 .5kU/L为临界点 ,单独测定血清CA12 5对诊断内异症的敏感性为 53.5% ,特异性为 56 .3%。单独测定EmAb诊断内异症的敏感性为 38.0 % ,特异性为 87.5%。联合二指标进行平行试验时 ,敏感性为 73.2 % ,特异性为 4 3.8% ;系列试验时敏感性为 18.3% ,特异性为 10 0 %。结论 :无论是单独还是联合运用血清CA12 To evaluate serum endometrial antibody(EmAb)and CA_125 in the diagnosis of endometriosis(EMs),eighty_seven inpatients who were ready to receive laparoscopy of pelvic operation served as the object of study.Of the 87 cases,71 with EMs were served as the treatment group,and 16 with pelvic inflammatory masses as the control group.Serum concentrations of CA_125 and EmAb were examined before operation,and an optimal critical point of CA_125 was established by Receiver Operator Characteristic Curve(ROC).Results showed that the difference of CA_125 in the two groups was not significant(P> 0.05 ).There was no positive correlation between CA_125 level and AFS classification of EMs(P> 0.05 ).At the optimal critical point of 12.5 kU/L,serum CA_125 had a sensitivity of 53.5% and a specificity of 56.3% in the diagnosis of EMs,and EmAb had a sensitivity of 38.0% and a specificity of 87.5% .The sensitivity and specificity by parallel tests were 73.2% and 43.8% respectively,and by serial tests 18.3% and 100% respectively.It is concluded that no matter using alone or in combination,serum CA_125 and EmAb cannot achieve good sensitivity or specificity for the diagnosis of the disease.
