摘要
目的预防老年胃肠手术患者切口感染。方法对132例老年胃肠手术患者切口感染情况进行回顾性分析;发生手术切口感染的132例患者为观察组,未发生手术切口感染的1375例患者为对照组。结果 1507例患者中发生医院感染332例,其中切口感染132例,感染率为8.76%,占发生术后医院感染总发生率的39.76%,引起切口感染的相关因素与患者的营养状况、合并糖尿病、手术时间、低蛋白血症、贫血等因素相关。结论切口感染是胃肠肿瘤外科手术患者较为常见的并发症之一,老年患者因其自身的特点,多伴有基础疾病,免疫力下降,更易发生切口感染;加强医院感染管理力度、注重老年患者的营养评估、有效控制血糖、加强术中无菌技术、及时补充白蛋白及输血,是降低老年胃肠手术患者切口感染率的关键。
OBJECTIVE To prevent the incision infections in the patients who underwent gastrointestinal surgery.METHODS The status of the infections in 132 elderly patients who underwent gastrointestinal surgery was analyzed retrospectively;totally 132 patients with surgical incision infections were set as the observation group,1375 patients without surgical incision infections as the control group.RESULTS Of 1507 patients investigated,nosocomial infections occurred in 332 cases,among which 132 cases were incision infections with the infection rate of 8.76%,and it accounted for 39.76% among the total incidence of nosocomial infections;the surgical incision infections were related with the nutritional status of the patients,combination of diabetes,operation duration,hypoalbuminemia,anemia and so on.CONCLUSION The incision infection is one of the common complications in the patients who undergo gastrointestinal cancer surgery,and the incision infections are prone to the elderly patients due to the self characteristics of their own including the combination of complications and low immunity;to strengthen the strength of management of hospital infections,focus on the evaluation of the nutrition of elderly patients,effectively control the blood sugar,intensify the intraoperative aseptic technique,supplement of albumin and blood transfusion in a timely manner are crucial to the decrease of the incision infections in the elderly patients undergoing gastrointestinal surgery.
