本文以犬复制心肌梗塞后猝死模型并进行心脏电生理检查。结果表明,程控刺激诱发的室速对再缺血室颤具有一定的预见性。心室不应期离散性(VDR)和梗塞范围(IS)与梗塞后快速室性心律失常的发生及猝死关系密切。VDR与IS呈正相关。
A canine model of chronic myocardial infarction was produced with two-stage occlusion of the left anterior descending coronary artery followed by partial reperfusion in 15 dogs with control group of 7 sham operated dogs. Cardiac electrophysiologic studies were performed on these dogs with programmed electric stimulation (PES). Subsequently, postinfarction sudden death was induced by acute ischemia of the posterolateral wall, using a continuous stimulation of 150μA direct current to the intima of the proximal circumflex artery. The predictive value of PES to postinfarction sudden death and the relation between ventricular arrhythmias induced by PES, abnormal electrophysiologic changes of infarct myocardium, infarct size (IS) and ischemic ventricular fibrillation (VF) were studied respectively. The results showed that ventricular tachycardia induced by PES was highly predictive of postinfarction sudden death. But VF induced by PES was not predictive. Both IS and dispersion of effective refractory period (VDR) were important determinants of PES- induced VT and ischemic VF. VDR correlated positively with IS (γ=0.84, P<0.01).
