摘要
目的研究复方黄柏液保留灌肠治疗溃疡性直肠炎疗效，并与柳氮磺砒啶栓进行对照。方法60例溃疡性直肠炎（其中男32例，女28例，平均年龄34．7岁，病程3个月-1年）随机分为A、B两组。A组应用复方黄柏液保留灌肠（100mll，每晚1次，共30d）治疗，B组应用柳氮磺吡啶栓剂（SASP，0．5g，每日2次，共30d）治疗。观察两组治疗后腹泻症状缓解情况、临床总疗效，临床症状缓解情况，肠镜缓解情况，组织学缓解情况。结果腹泻症状缓解情况：与B组比较，A组患者腹泻症状恢复正常时间（11．2±3．8，VS17．7±5．1d）及黏液脓血便消失时间（8．9．4±4．6，VS14．2±4．5d）明显缩短（P〈0．05）。临床总疗效：A组（完全缓解20例，有效9例，无效1例）显著优于B组（完全缓解12例，有效15例，无效3例，P〈0．05）。临床症状缓解情况：A组（完全缓解20例，部分缓解9例，无效1例）显著优于B组（完全缓解12例，部分缓解15例，无效3例，P〈0．05）。肠镜缓解情况：A组（完全缓解20例，部分缓解9例，无效1例）显著优于B组（完全缓解12例，部分缓解15例，无效3例，P〈0．05）。组织学缓解情况：A组（完全缓解19例，部分缓解10例，无效1例）显著优于B组（完全缓解11例，部分缓解16例，无效3例，P〈0．05）。胃肠道反应情况：A组未见不良反应，B组肝功能损伤、水样大便次数增多、皮疹各2例。结论复方黄柏液保留灌肠治疗溃疡性直肠炎安全有效。
Objective To evaluate the therapeutic effects of retention enema with Compound Phellodendron liq-uid in the treatment of ulcerative proctitis. Methods Sixty ulcerative proctitis patients were randomly divided into two groups：Group A compound phellodendron liquid retention enema group（ n = 30）and Group B Sulfasalazine supposito-ries （ n = 21 ）. Clinical effects and safety were compared in the two groups. Results Compared with group B, Group A had an obviously shorter diarrhea recovery time [ （ 11.2 ± 3.8 ） d vs （ 17.7 ± 5.1 ） d, P 〈 0.05 ] ; a shorter extinction time about the bloody diarrhea and mucus in stools [ （ 8.9 ± 4.6 ） vs （ 14.2 ±4.5 ） d, P 〈 0.05 ]. The overall clinical ef-ficacy of group A（ complete remission in 20 cases, improvement in 9 patients, ineffective in one case ）was better than that in group B（complete remission in 12 cases, improvement in 15 patients, ineffective in 3 cases, P 〈 0.05 ）. The clinical symptoms relieve of group A（ complete remission in 20 cases, partly in 9 patients, ineffective in one case）was better than that in group B（ complete remission in 12 cases, partly in 15 patients, ineffective in 3 cases, P 〈 0.05 ）. The colonoscopic remission of group A （complete remission in 20 cases, partly in 9 patients, ineffective in one case） was better than that in group B （ complete remission in 12 cases, partly in 15 patients, ineffective in 3 cases, P 〈 0. 05 ）. The histologic remission of group A （complete remission in 19 cases, partly in 10 patients, ineffective in one case） was better than that in group B （complete remission in 11 cases, partly in 16 patients,ineffective in 3 cases,P 〈 0.05 ）. The side effects of gastrointestinal tract in group A were fewer than that of group B except for frequency of wa-tery diarrhea. Conclusion Retention enema with Compound Phellodendron liquid for patients with ulcerative proctitis is safe and effective.
