摘要
目的:应用电化学测菌法监测粘接性牙周夹板对老年牙周炎患者龈沟液中牙龈卟啉单胞菌和血链球菌定植的影响。方法:老年慢性牙周炎患者10例,应用电化学测菌法检测患者患牙和健康对照牙初诊时、牙周基础治疗后夹板固定前、固定后2周、固定后4周相应位点共128个位点龈沟液中的牙龈卟啉单胞菌和血链球菌,细菌培养和定量PCR作对照,记录相应观察期患牙的菌斑指数、牙龈指数、探诊深度和临床附着水平。结果:夹板固定期间患牙检测位点龈沟液中牙龈卟啉单胞菌和血链球菌的数量显著少于初诊时,与牙周基础治疗后夹板固定前相比差异无统计学意义;在健康对照牙检测位点,牙龈卟啉单胞菌的数量变化同患牙检测位点,而血链球菌的数量与固定前相比,夹板固定后血链球菌的数量增加。血链球菌与牙龈卟啉单胞菌的比值总体呈上升趋势。牙周临床指标有明显改善。结论:粘接性牙周夹板短期内对老年患者牙周微生态无显著不良影响。
Objective:To investigate the effect of adhesive periodontal splint on Porphyromonas gingivalis and Streptococcus sanguinis colonization in aged chronic periodontitis patients using electrochemical method established in previous study.Methods:The changes of P.gingivalis and S.sanguinis amounts in gingival crevicular fluid of 128 sites from 10 aged periodontitis patients restored with adhesive splints at the first visit,splinting before,fixed after 2 weeks and fixed after 4 weeks were quantified by electrochemical method.Bacterial culture and real-time quantitative PCR were used as further assistants.Furthermore,periodontal clinical parameters including plaque index,gingival index,probing depth and clinical attachment level of the teeth with periodontal disease in corresponding stages were evaluated.Results:The results showed that,regardless of disease sites or sites of healthy controls,the absolute number of Porphyromonas gingivalis and Streptococcus sanguinis decreased after splinting compared with at the initial visit,and no significant change for Porphyromonas gingivalis was observed during the entire observation period after splinting.For Streptococcus sanguinis,the absolute number increased after splinting in healthy sites.Periodontal clinical parameters improved obviously throughout experiment period.Conclusion:Adhesive periodontal splint with optimal design and sufficient preparation will not have significant adverse impact on periodontal microflora.
