摘要
目的:探讨性早熟女童血清E2及促性腺激素释放激素(GnRH)激发试验与子宫和卵巢形态改变的关系。方法:对36例性早熟女童进行盆腔B超检查,测量卵巢、子宫体积、卵泡最大直径,基础血清雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)的水平及GnRH激发试验后血清FSH、LH水平并进行比较。结果:不同卵泡最大直径组的基础血清E2,GnRH激发试验30 min及60 min LH/FSH值差异有统计学意义(P<0.05)。卵泡最大直径与基础血清E2和LH值,GnRH激发试验后30 min及60 min LH/FSH值呈正相关(P<0.05或P<0.01)。子宫容积与基础血清LH、E2及GnRH激发试验30 min及60 min LH/FSH值呈正相关(P<0.01)。卵巢容积与基础值LH及GnRH激发试验30 min及60 min LH/FSH值呈正相关(P<0.05或P<0.01)。特发性中枢性性早熟组与外周性性早熟组子宫容积及卵巢容积值差异有统计学意义(P<0.05)。结论:卵巢、子宫形态学检查对于筛选GnRH激发试验有重要意义。
Objective: To explore the relationship between serum estradiol (E2) levels, gonadotropin releasing hormone (GnRH) stimulation test and morphologic changes of ovary and uterus in girls with precocious puberty. Methods: Thirty-six girls with precocious puberty accepted GnRH stimulation tests and pelvic ultrasound examinations. The values of ovarian volume and follicular size, and basic serum levels of E2, follicle stimulating hormone (FSH), luteinizing hormone (LH), and levels of FSH and LH after GnRH stimulation tests were detected and compared. Results: There were significant differences in basic serum levels of E2, the ratio of LH/FSH 30 min and 60 min after GnRH stimulation test between two types of ovaries (P 〈 0.05). The maximal follicular diameter was positively coIrelated with the basic serum E2 and LH, and the ratio of LH/FSH 30 min and 60 min after GnRH stimulation test(P 〈 0.05 or P 〈 0.01). The volume of uterus was positively correlated with the se- rum levels of LH and E2, the ratio of LH/FSH 30 min and 60 min after GnRH stimulation test (P 〈 0.01). The volume of ovary was positively correlated with the basic serum LH level and the ratio of LH/FSH 30 min and 60 min after GnRH stimulation test (P 〈 0.05 or P 〈 0.01). There was a significant difference in the uterine volume and ovarian volume between two types of precocious puberty groups (P 〈 0.05). Conclusion: Pelvic ultrasound is important for the first sieve of GnRH stimulation test.
