摘要
目的了解烧伤创面非发酵菌感染株的耐药性。方法对2008年1月-2010年12月保存的非发酵菌创面株经复苏、再鉴定后统一用K-B法作药敏试验。结果 334株非发酵菌中最多的是铜绿假单胞菌(PAE)、鲍氏不动杆菌(ABA)和嗜麦芽寡养单胞菌(SMA),其他细菌较少;该3种非发酵菌对氨苄西林的耐药率为92.1%～96.1%,对哌拉西林的耐药率为71.4%～81.6%,SMA对碳青霉烯类抗菌药物的耐药率高达94.7%,PAE和ABA分别为32.7%、37.2%,第三、四代头孢菌素类药物的耐药率为41.5%～72.9%,表明多数细菌属多药耐药株;敏感性较高的抗菌药物仅有头孢哌酮/舒巴坦、环丙沙星和阿米卡星。结论铜绿假单胞菌、鲍氏不动杆菌和嗜麦芽寡养单胞菌是烧伤创面感染最常见的非发酵菌,由于该类细菌耐药率严重,临床应加大抗菌药物的管理力度,提倡合理、有序、科学地使用抗菌药物,以达到预防和控制耐药菌产生、播散之目的。
OBJECTIVE To study the drug resistance of nonfermentative bacteria isolated from burn wounds.METHODS Nonfermentative isolates preserved from Jan.2008 to Dec.2010 were recovered and re-identified,and then drug susceptibility testing were performed by K-B methods.RESULTS Pseudomonas aeruginosa（PAE）,Acinotobacter baumannii（ABA） and Strenotrophomonas maltophilia（SMA） were most common among 334 strains of nonfermentative bacteria and the drug resistance rate to ampicillin was 92.1%-96.1%,the rate to piperacillin was 71.4%-81.6%.The drug resistance rate of SMA to carbapenem reached to 94.7%.The rates of PAE and ABA to carbapenem were 32.7% and 37.2%,respectively.The rates to the third and fourth generations of cephalosporines were between 41.5% and 72.9%,which indicated that most of nonfermentative bacteria were multidrug-resistant isolates.Of the antibiotics,only cefoperazone/sulbactam,ciprofloxacin and amikacin have the high susceptibility.CONCLUSION PAE,ABA and SMA are the most common nonfermentative bacteria isolated from burn wounds.In clinical practice,we should further strengthen the management of antibiotics,use antimicrobial agents reasonably,orderly and scientifically to prevent and control the emergence and spread of drug resistant strains.
