摘要
目的探讨小脑星形胶质瘤的诊断、鉴别诊断、治疗和并发症。方法回顾性分析28例小脑星形胶质瘤患者的一般情况、影像学特点、治疗方法以及并发症等。结果 28例患者中手术全切除18例,次全切除8例,部分切除2例;术后脑脊液漏3例,其中1例毛细胞型星形细胞瘤并发颅内感染死亡;一过性后组脑神经麻痹6例,共济失调10例,肺部感染2例,消化道出血1例;随访3个月至5年,术后3个月疗效评价,21例恢复良好,6例具有不同程度的神经功能障碍;随访达5年者13例,生存11例,5年生存率84.6%。结论小脑星形胶质瘤的预后相对较好,提高显微手术治疗技术,减少手术并发症可取得良好的疗效。
Objective To analyse the characteristics of diagnosis,treatment and operational complications of cerebellar astrocytomas.Methods General conditions,CT or MRI manifestations,treatments applied and complications of 28 cases with cerebellar astrocytomas were retrospectively analyzed.Results Total resection was performed in 18 cases,and near total resection in 8 cases,and partial resection in 2 cases.3 cases suffered cranial spinal fluid leaking after operation and 1 died from intracranial infection.Temporal cranial nerve palsy were found in 6 cases of,10 ataxia,2 pulmonary infection and 1 alimentary tract hemorrhage.Follow-up period lasts from 3 months to 5 years.As for the therapeutic effect evaluation 3 months after surgery,21 patients were good recovery,6 with functional impairment of different degrees.13 patients were followed up for 5 years and 11（84.6%）survived.Conclusion Cerebellar astrocytoma is a relatively benign intracranial tumor with good prognosis.Improvement of microsurgical resection and reducing of postoperative complications can achieve good therapeutic effects.
