微创经皮肾镜取石术治疗70岁以上老年人上尿路结石 被引量:4

Treatment of upper urinary tract calculi with minimally invasive percutaneous nephro- lithotomy in the elderly over 70 years

摘要 目的探讨微创经皮肾镜取石术治疗70岁以上老年人上尿路结石的疗效和安全性。方法2006年9月-2011年8月,应用微创经皮肾镜取石术治疗70岁以上老年人上尿路结石56例,对手术时间、结石清除率、手术并发症等临床资料进行回顾性分析。结果所有患者均一期成功建立皮肤肾脏通道,51例行一期碎石术,5例行二期碎石术;单通道碎石53例,双通道碎石3例。手术时间23~95min,平均(33±8.2)min。平均出血量为(151±55.7)ml,2例术中出血较多分别输血400和600ml,无术后出血严重需采用高选择性肾动脉栓塞治疗者,无气胸、腹腔脏器损伤等并发症发生。1例患者术后1天出现肺栓塞,经抢救后治愈。术后复查,52例结石取尽,清石率为92.9%。结论微创经皮肾镜取石术治疗70岁以上老年人上尿路结石是安全的、临床效果满意。 Objectives To evaluate the efficacy and safety of minimally invasive percutaneous nephrolithotomy (MPCNL) to treat upper urinary tract calculi in the elderly over 70 years. Methods 56patients 70 - 81 years old underwent IdPCNL from Sep 2008 to Aug 2011, the clinical data including the mean operating time, phase - I stone free - rate and complications were retrospectively analyzed. Results The pereutaneous renal access was successfully established in 56 patients, I period lithotripsy was performed in 51 cases, II period lithotripsy were performed in 5 cases. Single tract was used in 53 cases, double tract were used in 3 cases. The mean operating time was (33 ±8.2 )min, mean blood loss ( 151 ± 55.7 ) ml, 2 cases needed transfusion. No cases underwent hyperselective angloembolization for bleeding control, one case pulmonary embolism occurred and was cured. The phase - I stone free - rate was 92.9%. Conclusions The treatment of upper urinary tract calculi with percutaneous nephrolithotomy appears to be safe and efficacious in the elderly over 70 years.
