摘要
目的:观察大柴胡汤对梗阻性黄疸患者术后血清总胆红素(total bilirubin,TBIL)、谷丙转氨酶(alanineaminotransferase,ALT)及谷草转氨酶(aspartate aminotransferase,AST)的影响。方法:将手术后的366例梗阻性黄疸患者随机分为治疗组201例、对照组165例。治疗组在对照组治疗的基础上辅以口服大柴胡汤(柴胡30 g,大黄15 g,枳实12 g,黄芩15 g,半夏12 g,芍药15 g,大枣6枚,每日2次,100 mL/次),从术后第2天开始,连服7 d;对照组行梗阻性黄疸术后常规治疗,观察两组患者术后5～7,12～14 d血清TBIL,ALT及AST变化情况。结果:两组患者手术后血清TBIL,ALT及AST水平均明显下降,与手术前相比差异有统计学意义(P<0.05);治疗组血清TBIL,ALT及AST降低明显,与对照组相比差异有统计学意义(P<0.05)。两组手术后5～7 d,12～14 d血清TBIL,ALT,AST,对照组TBIL(138.3±20.4)μmol.L-1,ALT(124.5±17.9)U.L-1,TBIL(128.1±17.9)μmol.L-1,ALT(120.0±17.3)U.L-1,AST(54.5±4.9)U.L-1;治疗组TBIL(132.6±18.7)μmol.L-1,ALT(119.1±17.7)U.L-1,TBIL(121.0±17.2)μmol.L-1,ALT(114.3±17.1)U.L-1,AST(53.1±5.0)U.L-1,两组相比有显著性差异(P<0.05)。结论:大柴胡汤能有效降低梗阻性黄疸患者术后血清TBIL,ALT及AST,起到促进肝脏功能恢复的作用。
Objective：To observe the influence of Dachaihu Tang on serum concentration of total bilirubin（TBIL）,alanine aminotransferase（ALT）and aspartate aminotransferase（AST）in Post-operative patients with obstructive jaundice surgery.Method： 366 cases of post-operative patients with obstructive jaundice were randomly divided into treatment group of 201 cases and control group of 165 cases.The control group received conventional treatment after surgery of obstructive jaundice,treatment group was supplemented with high daisaikotofrom the second day after surgery,lasting a total of seven days,the serum concentrations of TBIL,ALT and AST were observed after 5-7 d and 12-14 d in both group.Result：The serum concentrations of TBIL,ALT and AST in both group were significantly decreased compared with pre-operation.Compared with the control group there were statistically significant differences（P0.05）.The serum concentrations of TBIL,ALT and AST in treatment group decreased significantly,compared with the control group significantly（P0.05）.Conclusion： Dachaihu Tang can effectively reduce the serum concentration of TBIL,ALT and AST,playing an active role in liver protection.
