摘要
目的评估第二代双源CT前门控大螺距冠状动脉成像(Flash模式)中ECG呈现的"不稳定波形"对图像质量的影响。方法回顾分析Flash模式冠状动脉成像的73例连续病例,与采集数据前ECG波形比较,观察采集数据的单个R-R间期中波形变化。采用4级法评估图像质量。结果组1(30例)为"不稳定波形"组,组2(43例)为"稳定波形"组。"不稳定波形"发生于数据采集时和(或)数据采集完成后,表现为基线抬高或下移,P波或QRS波群倒置、宽大畸形,T波~P波之间无命名的波形,波形呈M形、W形、V形或∧形,但R-R间期无变化。以血管段及患者为单位,两组间图像质量评分的差异均无统计学意义(P均>0.05);可评估率的差异无统计学意义(P均>0.05)。结论前门控大螺距冠状动脉成像时,仅在图像采集R-R间期呈现的ECG"不稳定波形"可能为伪差,不对图像质量产生明显影响。
Objective To analyze the characteristics of "unstable waves" in ECG, and to evaluate the impact of "unstable waves" on image quality during coronary CT angiography using the prospective ECG-triggered high pitch spiral mode (Flash mode) with the second generation dual source CT (DSCT). Methods Coronary CTA data of DSCT Flash mode in 73 consecutive patients were retrospectively analyzed. The changes of the wave shape in R-R interval were compared with that before the acquisition. Image quality was evaluated and graded to 4 scales. The comparison was made between group 1 ("unstable waves" group) and group 2 ("stable waves" group). Results Thirty and 43 patients were involved in group 1 and group 2, respectively. ECG "unstable waves" usually located in and after the phase of data acquisition and behind T wave. The "unstable waves" presented as the baseline drifting, and the enlarged, elevated, inverted P or QRS waves, or as a new abnormal wave. The morphology of "M, W, V or ∧ " shape were observed, but R-R interval did not changed. There was no significant difference between the two groups both on the basis of patients and segments for image quality scores (all P〉0.05), nor for coronary evaluability (all P〈0.05). Conclusion The "unstable waves" in ECG during data acquisition might be artifacts, and it is not affect the image quality.
