摘要
目的探讨转录因子T-bet和GATA-3在过敏性紫癜(HSP)患儿发病机制中的作用。方法本院儿科2009年2月-2010年2月收治急性期HSP患儿46例(HSP组)以及健康对照儿童30例(健康对照组)。采用SYBR GreenⅠ实时荧光定量PCR方法检测其外周血单个核细胞T-bet mRNA、GATA-3 mRNA的表达。应用Luminex-100液相芯片技术检测其血浆细胞因子IL-2、IL-4、IFN-γ水平。结果 HSP组患儿外周血单个核细胞T-bet mRNA相对表达水平(53.98±35.79)低于健康对照组(181.56±96.90)(P<0.001)。GATA-3 mRNA相对表达水平(964.30±655.18)高于健康对照组(78.09±57.20)(P<0.001)。HSP组血浆细胞因子IL-2[(16.54±7.38)ng.L-1],IFN-γ[(11.31±8.02)ng.L-1]的表达显著低于健康对照组[(35.73±22.66)ng.L-1,(30.67±21.29)ng.L-1](P<0.001,0.05),IL-4[(74.66±26.15)ng.L-1]的表达高于健康对照组[(51.81±27.76)ng.L-1]。IFN-γ水平与T-bet mRNA表达呈正相关(r=0.882,P<0.01);IL-4水平与GATA-3 mRNA的表达呈正相关(r=0.886,P<0.01)。结论 HSP患儿急性期存在Th1/Th2失衡,主要表现为Th2优势活化,该失衡与其特异性转录因子T-bet mRNA表达降低、GATA-3 mRNA表达增高有关,本研究为阐明Th1/Th2失衡的分子机制之一提供了实验依据。
Objective To explore the role of transcription factors T - bet and GATA - 3 in pathogenesis of Henoch - Schonlein purpura (HSP) in children. Methods In 46 patients with HSP in the acute phase and the control group( n = 30 ), the expressions of T - bet mRNA and GATA -3 mRNA were detected by real -time PCR using SYBR Green I , and in the plasma the expression of cytokine IL -2, IL -4 and IFN - γ were detected by Luminex - 100. Results The relative level of T - bet mRNA in peripheral blood mononuclear cells of patients with HSP( 53.98 ± 35.79 ) were significantly lower than those in the control group ( 181.56 ± 96.90) ( P 〈 0.001 ). The relative levels of GATA - 3 mRNA in peripheral blood mononuclear cells of patients with HSP( 964.30 ± 655.18 )were significantly higher than those in the control group(78.09 ± 57.20) (P 〈 0. 001 ). The expressions of plasma cytokines IL -2, IFN - 3' in HSP were ( 16.54 ± 7.38 ) ng .L^-1, ( 11.31± 8.02) ng.L^-1, respectively. The expressions of eontrol group were (35.73 ± 22.66 )ng.L^-1, (30.67±21.29 )ng.L^-1, respeetively. The expressions of HSP were lower than those in the eontrol group. There was signifieanfly statistical signifieanee (P 〈 0. 001,0.05 ) between HSP group and control group. The expression of plasma cytokine IL - 4 in HSP [ (74.66 ± 26.15 )ng. L^-1 ] were signifieantly higher than that in the control group [ (51.81 ± 27.76) ng. L^-1 ] ( P 〈 0.01 ). Conclusions Th1/Th2 imbalance exists in the acute phase of children HSP, especially predominant activation of Th2 ,which is related to the abnormal expression of transcription factors T- bet mRNA, GATA -3 mRNA. This study interprets the imbalance of Th1/Th2 from one of molecular mechanisms.
