摘要
目的探讨高效价冷凝集对血常规检测结果的影响及消除方法。方法对有高效价冷凝集素的血常规检标本分别采用未处理直接检测法、37℃水浴法、血浆置换法,利用sysmex XE-2100血细胞分析仪进行检测,并与传统的手工显微镜计数项目WBC、RBC、PLT及血涂片分类结果进行分析比较。结果有高效价冷凝集素的血常规检标本水浴前、后PLT、Hb、MPV和白细胞分类结果与实际相符,其它多项检测结果与实际不符,尤其是RBC、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)相差十分悬殊,且WBC、RBC、PLT散点图和直方图均明显异常并有相应报警;而置换血浆法除PLT结果偏低外其它检测参数,均与手工显微镜计数项目相符合,散点图和直方图正常,报警消失。结论对有高效价冷凝集素的血常规标本的检测,无论是未处理直接检测,还是经过37℃水浴后检测,除Hb、MPV、白细胞分类结果与实际吻合外,高效价冷凝集素对多项检测参数有严重干扰;而置换血浆法可以有效排除这些干扰,因置换过程会造成PLT的丢失,故可采用手工计数或者直接上机检测的PLT结果报告。
Objective To investigate the effect of high titer cold agglutinin on hematology analysis and its eliminate measures.Methods The specimen of the high titer cold agglutinin was tested respectively with 37℃water bath in front and back,and exchange plasma by the Sysmex XE-2100 and the results were analyzed.Results In 37℃water bath in front and back The hematology analyzer can be displayed the most parameters is incorrectness except PLT、Hb、MPV and leukocyte differential count.Particularly,the results of RBC、MCH、MCHC showed significant difference between 37℃water bath in front or back and in fact.The hematology analyzer also can be displayed the abnormal scatter and histogram for WBC、RBC、 PLT and gave corresponding alarm.The other way round,the results of exchange plasma can be displayed the most results is correctness except PLT,The corresponding alarm were disappeared.Conclusion The specimen of the high titer cold agglutinin has significantly effect on results of sysmex XE-2100 hematology analyzer.In 37℃water bath in front and back can＇t eliminate the effect except Hb、MPV and leukocyte differential count.Therefore the suspected cold hemagglutination specimen should be exchange plasma to eliminate the effect and tested again.If the PLT lost more,you can use the result of direct tested.
