摘要
目的探讨应用高强度聚焦超声（HIFU）配合局部专用药物治疗女性外阴上皮内非瘤样病变的方法及疗效。方法研究组共376例，分为单独应用HIFU治疗的75例（A组）和HIFU合并局部专用药物治疗的301例患者（B组）。以同期应用传统药物治疗的56例患者为对照组。观察2组的治疗情况，并以外阴瘙痒评分法、外阴白色病变面积计算法、四象限比较法和四度皮肤弹性评估法客观判别疗效。结果A、B2组对各类外阴上皮内非瘤样病变的瘙痒症状有满意疗效，明显优于对照组（P〈0．05），以B组疗效最佳，治愈率83．4％（252／301）。A、B组对白色病变有效率均为100％，A组治愈率为64．0％（48／75），B组为72．1％（217／301）。B组白色病变面积平均由（15．37±8．73）cm^2减小为（6．73±3．01）cm^2、A组（11．40±6．57）cm^2减小为（4．21±1．69）cm^2、对照组由（8．73±4．26）cm^2减小为（7．15±3．43）cm^2，统计学显示研究组（A和B组）治疗后白色病变区面积减小均有统计学意义（P〈0．01）。研究组对外阴皮肤白变转归和皮肤弹性的疗效均明显优于对照组（P〈0．05）。结论HIFU辅以专用药物对于女性外阴上皮内非瘤样病变有良好疗效，并具有安全、无创等优点。
Objective To investigate the therapeutic methods and effect of high intensity focused ultrasound （HIFU） with special drugs for nonneoplasia epithelial disorders of skin and mucosa of vulva by. Methods The study group had three hundreds and seventy-six patients with nonneoplasia epithelial disorders of skin and mucosa of vulva. The patients were divided into two groups ： A-group （ single treated by HIFU） with 75 patients and B-group （ HIFU with drugs） with 301 patients. Fifty-six patients with same diseases treated singlely by traditional drugs were as control group. Results The curative rate of reliveving-pruritus, lichen sclerosus and vulvar leucoplakia in studing group （A and B） was statistically more effective in control group（ P 〈 0. 05 ）. The curative rate of pruritus and vulvar leueoplakia was 83.4% （252/301） and 72. 1% （217/301）, respectively, better than that in the A-group which was 64.0% （48/75）. The area of vulvar leucoplakia statistically decreased after treatment （ P 〈 0.05 ）. Conclusion HIFU combined with special drugs was a safe, effective and repeatable method for the treatment of nonneoplasia epithelial disorders of skin and mueosa of vulva.
