CT and MR Imaging Features of Subependymomas

摘要 目的:探讨室管膜下瘤的CT及MRI影像学征象,提高其诊断准确率。方法:回顾性分析27例经手术病理证实的室管膜下瘤患者的临床、病理和影像学资料。结果:室管膜下瘤是神经系统良性肿瘤,发病年龄平均40.7岁,具有一定特征性:①肿瘤多位于侧脑室内,第四脑室、颈髓或颈胸髓、脑实质内亦可发生。②肿瘤多呈类圆形、椭圆形或分叶状,髓内的多呈条状,边界清晰,病灶内常伴小的囊变,少数有细小的砂粒样钙化。③CT平扫肿瘤呈低或等密度,MRI平扫T1WI呈等或稍低信号,T2WI及FLAIR呈高信号,DWI呈低信号;1H-MRS显示为低级别良性肿瘤的代谢波峰。肿瘤内的小囊变区呈更低密度及T1WI更低、T2WI更高信号影,有时瘤内可见流空血管影。④MRI增强扫描病灶一般无或轻度强化,具有特征性。结论:充分认识室管膜下瘤的CT及MRI影像学特征,可以提高术前诊断正确率。 Purpose: To study the CT and MRI features of subependymomas.Methods: The clinical, pathological and medical imaging data in 27 cases of subependymomas proved by pathological diagnosis were reviewed and analyzed. Results: Subependymomas were benign and slow - growing tumors occurred most commonly in middle - aged male, their features included: (1) Tumors arose most frequently in the lateral ventricle, followed by fourth ventricle, spinal cord and brain parertchyma. (2) The tumors were circular or oval solid mass with clear borders, there were microcysts and occasionally small calcifications in the tumors. (3)The tumors showed isodense or hypodeuse on unenhanced CT, slightly hypo or isointense signal on T1 weighted imaging, hyperintense signal on T2 weighted imaging and FLAIR imaging, hypointense signal on diffusion weighted imaging. The tumors showed lowgrade neoplasm's metabolic features on ^1H - MR spectroscopy imaging findings. (4) No to mild contrast enhancement were mostly found in tumors. Conclusion: Subependymomas have characteristic CT and MRI features, which are very helpful in pre - operative diagnosis and in post - operative evaluation.
