摘要
[目的]探讨2型糖尿病患者合并尿路感染的病原菌分布及耐药情况,以指导临床合理用药。[方法]对212例2型糖尿病合并尿路感染患者进行细菌学培养,并对其中的阳性标本进行病原学鉴定及药敏实验。[结果]212株糖尿病患者尿路感染病原菌依次为:大肠埃希菌126株,占59.4%;其次为粪肠球菌27株,占12.7%;肺炎克雷伯菌19株,占9.0%;凝固酶阴性葡萄球菌16株,占7.5%;铜绿假单胞菌13株,占6.1%;白色假丝酵母菌11株,占5.2%。病原菌除对亚胺培南、美罗培南和阿米卡星较敏感外,对其他常用抗菌药物均产生了较大的耐药性。[结论]糖尿病病人尿路感染细菌培养阳性率高,耐药率高,临床应重视糖尿病疑为尿路感染的尿培养及药敏检测,以指导临床合理用药。
[Objective]To observe the pathogen distribution and the drug resistance isolated from diabetes patients with urinary tract infection,to guide rational drug use.[Methods]Pathogenic bacteria isolated from specimens of 236 cases with urinary tract infections were identified and drug susceptibility tests were performed.[Results]Among 212 strains of pathogenic bacteria,Escherichia coli rated the top one,59.4%.Then Enterococcus rated the second and accounted for 12.7%.Klebsiella pneumoniae in 19 strains,accounting for 9.0%;coagulase-negative staphylococci in 16 strains,accounting for 7.5%;Pseudomonas aeruginosa in 13 strains,accounting for 6.1%;Candida albicans in 11 strains,accounting for 5.2%.The isolates were susceptible to imipenem and amikacin and showed higher drug-resistance to the other common antibiotics.[Conclusion]As a result of high positive isolation rate and high resistance rate in diabetes patients with urinary tract infection,clinical doctors should pay more attention to the middle-segment urine culture and drug resistance.
