摘要
目的通过分析初治肺结核患者CD3+、CD4+、CD8+T淋巴细胞计数结果,透视初治肺结核患者的细胞免疫状况。方法对111例初治肺结核患者的全血样本进行CD3+、CD4+、CD8+T淋巴细胞计数(/μl),依性别和<20、21～、31～、41～、51～、61～、71～、>81岁等8个年龄组别对计数结果进行分析。结果 (1)CD3+T淋巴细胞计数:男性各年龄组别结果分别为986±473、892±287、954±388、893±529、761±432、758±336、714±372和332±183,低于正常值者分别占66.67%、50.00%、87.50%、68.75%、72.73%、81.82%、85.71%和66.67%(总计72.29%);女性各年龄组别结果分别为1418±0、902±354、1039±515、712±399、584±271、778±234、723±234和107±0,低于正常值者分别占0、66.67%、50.00%、100.00%、100.00%、100.00%、75.00%和100.00%(总计75.00%)。(2)CD4+T淋巴细胞计数:男性各年龄组别结果分别为604±328、482±186、565±231、535±340、454±275、422±221、443±230和270±129,低于正常值者分别占66.67%、35.71%、37.50%、37.50%、36.36%、63.64%、57.14%和66.67%(总计46.99%);女性各年龄组别结果分别为896±0、474±202、626±335、446±311、333±208、426±182、460±280和66±0,低于正常值者分别占0、33.33%、33.33%、100.00%、100.00%、100.00%、75.00%和100.00%(总计64.29%)。(3)CD8+T淋巴细胞计数:男性各年龄组别结果分别为309±132、315±124、298±141、300±209、227±122、282±133、231±151和46±35,低于正常值者分别占83.33%、57.14%、100.00%、81.25%、81.82%、63.64%、85.71%和66.67%(总计77.11%);女性各年龄组别结果分别为463±0、323±134、314±169、189±49、196±104、286±91、220±149和28±0,低于正常值者分别占0、66.67%、33.33%、100.00%、100.00%、100.00%、100.00%和100.00%(总计75.00%)。结论初治肺结核患者多有CD3+、CD4+、CD8+T淋巴细胞计数偏低,尤以老年龄组患者为甚,初治肺结核患者的低细胞免疫状态值得临床关注。
Objective To survey cell - mediated immune status of the untreated patients with pulmonary tuberculosis by counting the amount of CD3 ＋ , CD4＋ , CD8 ＋ T lymphocyte. Methods CD3 ＋ , CD4＋ , CD8 ＋ T lympbocytes （cell amount/ul） of 111 cases were counted, and these data were analyzed according to sex and age groups（ 〈 20,21 ,31 - ,41 - ,51 - ,61 -,71 - , 〉 81 years）. Results （1）For CD3＋ T lymphocyte, the amounts were respectively 986 ± 473,892 ±287,954 ± 388,893 ± 529,761 ± 432,758± 336,714 ± 372 and 332 ±183 for male cases and 1 418 ±0,902 ±354,1 039 ±515,712 ±399,584 ±271,778 ±234,723±234 and 107 ±0 for female cases. The male patients that the amount of CD3 ＋ T lymphocyte was lower than normal value accounted for 66. 67% ,50. 00% ,87.50%, 68.75% ,72. 73% ,81.82% ,85.71% and 66. 67% respectively among the various age groups and 72. 29% for all male. But, they were 0,66. 67% ,50. 00% ,100. 00% ,100.00% ,100. 00% ,75.00% , 100. 00% and 75.00% respectively for female. （2） For CD4 ＋ T lymphocyte, the amounts were respectively 604 ± 328,482 ± 186,565 ± 231, 535 ± 340,454 ± 275,422 ± 221,443 ± 230 and 270 ± 129 for male cases and 896 ± 0,474 ± 202,626 ± 335,446 ± 311,333 ± 208,426 ± 182,460 ± 280 and 66 ± 0 for female cases. The male patients that the amount of CD4＋ T lymphocyte was lower than normal value accounted for 66.67% , 35.71% , 37.50% , 37.50% , 36. 36% , 63.64%, 57.14% and 66. 67% respectively among the various age groups and 46. 99% for all male. But, they were 0, 33.33% ,33.33% , 100. 00% , 100. 00% , 100. 00% ,75.00% , 100. 00% and 64. 29% respectively for female. （ 3 ） For CD8＋ T lymphocyte, the amounts were respectively 309 ± 132,315 ± 124,298 ± 141,300 ± 209,227 ± 122,282 ± 133,231 ± 151 and 46 ± 35 for male cases and 463 ±0,323 ± 134,314 ± 169,189 ±49,196 ± 104,286 ±91,220 ± 149 and 28 ± 0 for female cases. The male patients that the amount of CD8 ＋ T lymphocyte was lower than normal value accounted for 0,66. 67% ,33.33% ,100. 00% ,100. 00% ,100. 00% ,100. 00% and 100. 00% respectively among the various age groups and 77.11% for all male. But, they were 0,66. 67% ,33.33%, 100.00%, 100. 00%, 100. 00% ,100. 00% , 100. 00% and 75.00% respectively for female. Conclusion The most of untreated patients with pulmonary tuberculosis have inadequate amounts of CD3 ＋ ,CD4 ＋ ,CD8 ＋ T lymphocytes, especially aged cases. It is important that low cell -mediated immune status should he pay attention to the untreated patients with pulmonary tuberculosis.
