摘要
目的观察宫腔镜在稽留流产清宫术中的应用价值。方法将116例符合病例入选标准的稽留流产患者随机分成观察组和对照组,2组均给予米非司酮及米索前列醇,对照组直接清宫,观察组在宫腔镜下清宫。比较2组流产成功率、手术时间、术中出血量、术后阴道流血时间、住院时间、术后宫腔残留再次清宫例数及发现畸形子宫例数。结果观察组完全流产率100.00%;对照组完全流产率77.59%;不全流产率17.24%,总流产率94.83%。2组完全流产率及总流产率相比差异有统计学意义(P<0.05)。阴道流血量多于月经量者观察组有9例(15.52%),对照组有19例(32.76%),观察组出血量较少,2组相比差异有统计学意义(P<0.05)。手术时间、手术出血量2组相比差异无统计学意义(P>0.05),术后流血时间、妊娠物残留例数、再次清宫例数、子宫畸形例数等2组相比差异有统计学意义(P<0.05)。结论宫腔镜下清宫用于稽留流产有很大的临床价值。
Objective To observe the application value of hysteroscopy in missed abortion curettage.Methods One hundred and sixteen patients met the inclusion criteria of the missed abortion cases were randomly divided into observation group and control group.The two groups were treated with mifepristone and misoprostol.Curettage was performed directly on the patients of control group.Hysteroscopy was applied in observation group.The success rate of miscarriage,operation time,intraoperative blood loss,postoperative vaginal bleeding,hospitalization time,the number of cases with second curettage for postoperative residual uterus,and the number of cases with uterine malformation were compared between the two groups.Results The complete abortion rates in observation group and control group were 100.00% and 77.59%,respectively.The incomplete abortion rate was 17.24%,and the total abortion rate was 94.83%.There were statistically significant differences in the complete abortion rate and the total abortion rate between the two groups（P0.05）.In observation group,9 patients＇ vaginal bleeding was more than menstrual bleeding,while that in control group was 19.The vaginal bleeding in observation group was less than that in control group,and the difference was statistically significant（P0.05）.No statistically significant difference was found in operation time and intraoperative blood loss between the two groups（P0.05）.There were statistically significant differences in postoperative vaginal bleeding,the number of cases with postoperative residual uterine,the number of cases with second curettage,and the number of cases with uterine malformation between the two groups（P0.05）.Conclusion The hysteroscopic curettage has great clinical value in missed abortion.
