0.062 5%与0.1%布比卡因在硬膜外分娩镇痛中的效果比较

摘要 目的将0.062 5%布比卡因应用于分娩镇痛,且与0.1%布比卡因的镇痛效果进行比较,进而评估在产程早期0.062 5%布比卡因是否适用。方法将60例产妇随机分为A组(0.1%布比卡因+0.000 2%芬太尼)和B组(0.062 5%布比卡因+0.000 2%芬太尼),观察局部麻醉药用量、患者生命体征、疼痛缓解情况、分娩结局和不良反应等。结果两组15 min时麻醉平面超过T10的产妇比例以及布比卡因总量的差异均无统计学意义(P值均>0.05),但B组需补充布比卡因的比例显著高于A组(P<0.05),B组有双下肢麻木感的产妇比例显著低于A组(P<0.05)。结论 0.062 5%布比卡因复合芬太尼适用于产程早期的分娩镇痛。 Objective To compare the analgesic effects of 0.062 5% and 0.1% bupivacaine on labor pain,so as to evaluate whether 0.062 5% bupivacaine is suitable for early labor.Methods Totally 60 parturients in early active labor were recruited and randomized into two groups.All subjects received analgesic drugs via a lumbar epidural catheter.Group A: 0.1% bupivacaine plus 0.000 2% fentanyl;Group B: 0.062 5% bupivacaine plus 0.000 2% fentanyl.The analgesic requirement,visual analogue scale(VAS),motor block and obstetric outcome were compared between the two groups.Results No significant differences were found between the two groups in the total bupivacaine consumption and the percentages of subjects with anesthesia plane over T10 at 15 min(both P0.05).More patients in group B needed additional supplements(P0.05).Two groups achieved the same analgesic effect(P0.05),but more patients in group B had second stage maternal lower limb numbness compared with group A(P0.05).Conclusion It is indicated that 0.062 5% bupivacaine with fentanyl can be used for early labor epidural analgesia.
