健侧腓肠神经营养皮瓣桥式交叉修复足部软组织缺损 被引量:2

REPAIR OF SOFT TISSUE DEFECT IN FOOT WITH SURAL NEUROVASCULAR FASCIOCUTANEOUS ISLAND FLAP IN UNINJURED SIDE

摘要 目的探讨应用健侧腓肠神经营养皮瓣桥式交叉修复患侧前足大面积软组织缺损手术方式的可行性,总结其临床疗效。方法自2006年7月—2009年11月,对足部压砸伤术后远端坏死12例患者应用健侧腓肠神经营养皮瓣修复足部远端大面积软组织缺损(清创后软组织缺损范围为12cm×8 cm~16 cm×12 cm)。结果术后皮瓣全部成活,无皮缘坏死病例。随访3~26个月,中位数18个月,患肢正常步态行走,皮瓣耐磨,外观满意,感觉部分恢复。结论应用健侧腓神经营养皮瓣桥式交叉修复足部远端大面积软组织缺损,不牺牲肢体主要血管,皮瓣切取方便、成活率高,质地良好,吻合神经后皮瓣能恢复感觉,适合足前部负重区软组织缺损的修复。 Objective To explore the clinical feasibility of sural neurovascular fasciocutaneous island flap in uninjured side on the repair of soft tissue defect in foot and summarize its therapeutic effect. Methods From July 2006 to November 2009,12 patients with soft tissue defects in foot were treated with sural neurovascular fasciocutaneous island flap in uninjured side. All the patients had a crush injury in foot which were necrotic after the frist operation. The soft tissue defects after the debridement of necrotic tissue ranged in area from 12cm × 8cm to 16cm × 12cm. Results All cases of the flaps survived. The distal skin necrosis did not occur in flaps. The follow - up of 3 to 26 months revealed that the patients had a normal gait. The flaps were sensible and wear - resist. Conclusion Repairing of soft tissue defect in foot with sural neurovascular fasciocutaneous island flap in uninjured side is easy, without any sacrifice of the major blood vessel. The flaps have a high survival rate, a good sense and a resistance to wearing. It is a good way to repair soft tissue defect in foot.
