摘要
[目的]比较临床应用股骨近端锁定板(locking proximal femur plates,LPFP)和动力髋螺钉(dynamichip screw,DHS)治疗老年股骨转子间骨折的手术过程及疗效。[方法]回顾性分析老年股骨粗隆间骨折84例患者的临床资料,其中41例采用LPFP治疗,43例采用DHS治疗。分析比较两种术式平均手术时间、术中出血量、术中术后输血量、住院时间以及术后功能恢复方面的差异。[结果]两组病例在手术时间、术中出血量、术中术后输血量平均值方面的差异无统计学意义。平均住院时间分别为20.88 d(LPFP)和27.58 d(DHS)。术后髋关节功能恢复的优良率分别为85%(LPFP)和78.57%(DHS)。[结论]在治疗老年股骨粗隆间骨折方面,LPFP患者明显较DHS患者住院时间短,且髋关节功能恢复的优良率方面LPFP也略有优势。
[Objective]To compare the operative processes and analyze the clinical effect of locking proximal femur plates（LPFP） and dynamic hip screw（DHS） for treating senile intertrochanteric fractures.[Method]Eighty-four patients with intertrochanteric fractures in elderly patients were reviewed.Forty-one patients were treated with LPFP,including 16 males and 25 females with an average age of 79.83 years（range,63 to 91 years）.Fourteen cases were of type A1,19 of type A2 and 8 of type A3.Forty-three patients were treated with DHS,including 20 males and 23 females with an average age of 78.12 years（range,60 to 88 years）.Seventeen cases were of type A1,19 of type A2 and 7 of type A3.The operation time,intraoperative blood loss,blood transfusion,hospital stay time and hip function grade were recorded.[Result]There was no significant difference between the two groups in the mean operation time,intraoperative blood loss and blood transfusion（P0.05）.The hospital stay was 20.88±7.386 days in LPFP group and 27.58±13.110 days in DHS group（P0.05）.All patients were followed up for 8.6 months（range,6 to 18 months）.Postoperative hip function was better in LPFP group than in DHS group,but not significantly（P0.05）.[Conclusion]These results demonstrated that LPFP and DHS were favorable for treating senile femoral intertrochanteric fractures.LPFP had shorter hospital stay and better clinical results than in DHS group.
