摘要
目的:探讨显微线圈(microscopy coil,MC)MR高分辨率成像技术在膝关节内侧副韧带(medial collateral liga-ment,MCL)损伤诊断中的应用价值。方法:收集MCL损伤患者39例,分别使用膝关节线圈(knee-sense coil,KC)及47 mm MC进行膝关节MRI检查,对两种线圈成像图像进行评分,比较其成像质量有无差异;并以手术结果为标准,评价两种线圈诊断MCL损伤的敏感性、特异性和准确性。结果:Ⅰ级MCL损伤18条;Ⅱ级MCL损伤12条;Ⅲ级MCL损伤9条。MC组各序列图像能分别显示膝关节内侧副韧带及周围主要结构,39例MCL损伤中(Ⅰ级~Ⅲ级)主要结构均评为3或4分;KC组Ⅰ级MCL损伤评分为0或1分,Ⅱ级MCL损伤评分为1或2分,Ⅲ级MCL损伤评为3分。在诊断Ⅰ级MCL损伤中,MC组图像评分明显高于KC组,具有显著统计学意义,P<0.01;对Ⅱ级、Ⅲ级MCL损伤诊断MC组与KC组无显著统计学意义。MC组诊断MCL损伤的敏感性、特异性和准确性分别为:Ⅰ级MCL损伤为90.5%,94.9%和97.7%,Ⅱ级MCL损伤为90.3%,93.9%和96.7%,Ⅲ级MCL损伤90.0%,92.9%和97.0%;KC组诊断MCL损伤的敏感性、特异性和准确性分别为:Ⅰ级MCL损伤为87.5%,92.9%和91.7%,Ⅱ级MCL损伤为89.7%,92.5%和91.3%,Ⅲ级MCL损伤87.0%,92.5%和90.7%。结论:显微线圈可以清晰显示MCL,能准确诊断MCL损伤并进行分级,具有重要临床应用价值。
Objective:To explore the diagnostic values with microscopic coil(MC) MR high resolution imaging technology in knee Medial Collateral Ligament(MCL) injury. Methods:39 MCL injuries were examined with Knee-sense coil(KC) and 47 mm microscopic coil(MC) respectively.Then,the two kinds of coil imaging were scored and the differences in imaging quality were compared.Finally,the sensitivity,specificity,and accuracy with two coils in the diagnosis of MCL injury were evaluated in comparison to surgical findings.Results:There were 18 with MCL injury in gradeⅠ,12 in gradeⅡ,and 9 in grade Ⅲ.MC group can clearly display MCL and the main knee structure score was 3 or 4.In KC group,the MCL injury score was 0 or 1 in gradeⅠ,1 or 2 in gradeⅡ,and 3 in grade Ⅲ.In MC group,image score was significantly higher than KC group in gradeⅠMCL injury(P〈0.01),whereas,there was no significant statistical significance in gradeⅡ and gradeⅢ MCL injury.In MC group,the diagnostic sensitivity,specificity,and accuracy were 90.5%,94.9%,and 97.7% in grade Ⅰ,90.3%,93.9% and 96.7% in grade Ⅱ,and 92.9%,90.0% and 97.0% in gradeⅢ,respectively.Correspondingly,those parameters were 87.5%,92.9% and 91.7% in grade Ⅰ,89.7%,92.5% and 91.3% in gradeⅡ,and 87.0%,92.5% and 90.7% in gradeⅢ in KC group. Conclusion:The microscopic coil can accurately diagnose and grade MCL injury,thereby possessing important value in clinical applications.
