2型糖尿病同时合并冠心病及脑卒中的临床分析和研究

摘要 目的探讨2型糖尿病合并冠心病及脑卒中患者的临床特点。方法对46例糖尿病合并冠心病及脑卒中患者、同期42例单纯脑卒中患者及50例单纯冠心病患者进行回顾性分析,比较3组患者血脂水平、血糖水平、冠脉造影结果和颅脑CT或MRI等结果。结果糖尿病合并冠心病及脑卒中患者总胆固醇、三酰甘油、低密度脂蛋白、空腹血糖、餐后2h血糖、糖化血红蛋白显著高于单纯冠心病及单纯脑卒中患者,而高密度脂蛋白显著低于单纯冠心病及单纯脑卒中患者;冠脉造影显示糖尿病合并冠心病及脑卒中患者冠脉病变支数及评分显著高于单纯冠心病患者;颅脑CT或MRI检查显示,糖尿病合并冠心病及脑卒中患者以腔隙性梗死和多发性梗死为主,两组患者脑梗死差异有统计学意义(P>0.05)。结论严格控制血糖和血脂对预防和减少糖尿病并发脑卒中和冠心病的发生及预后具有重要的意义。 Objective To study the clinical features of in old patients with type 2 diabetes mellitus complicated with coronary heart disease and stroke.Methods 46 cases of old patients with type 2 diabetes mellitus complicated with coronary heart disease and stroke,42 cases of old patients with single coronary heart disease and 50 cases of old patients with single stroke were analyzed retrospectively.Blood lipid level,blood glucose level,coronary angiography results and CT or MRI results.Results Compared with single coronary heart disease group and single stroke group,TC,TG,LDL-C,FBB,2 hPBG and HbA1C were significanltly increased in patients with type 2 diabetes mellitus complicated with coronary heart disease and stroke,while HDLC was significanltly decreased.Coronary angiography revealed that number of diseased coronary vessels and score were significanltly higher that in single coronary heart disease group.CT or MRI result showed that lacunar infarction and multiple cerebral infarction were the main types in patients with type 2 diabetes mellitus complicated with coronary heart disease and stroke.Conclusion Control of blood glucose and lipid level is beneficial for the prevention of coronary heart disease and stroke in patients with type 2 diabetes mellitus.
