摘要
目的传统额窦手术方法创伤较大,文中探讨鼻内镜下筛泡前径路额窦开放术式微创治疗孤立性额窦炎的可行性、技术要点及疗效。方法 15例孤立性额窦炎患者,全身麻醉下行鼻内镜下筛泡前径路额窦手术,1例术中辅助额窦微钻定位额窦开口,术后常规随访评估疗效。结果术后随访6～24月,平均12.7月,12例患者术后头痛症状完全缓解,3例明显缓解,鼻内镜下额窦口开放良好完全上皮化12例,3例未能在鼻内镜下证实额窦开放,但患者均无主观症状。结论筛泡前径路额窦手术是治疗孤立性额窦炎的理想术式。
Objective Considering the obvious additional damage of traditional frontal sinus surgery,we explored the feasibility,key techniques,and curative effect of a novel endoscopic frontal sinus surgery via the route anterior to the ethmoid bulla for isolated frontal sinusitis.MethodsFifteen patients with isolated frontal sinusitis received endoscopic frontal sinus surgery via the route anterior to the ethmoidal bulla.Frontal minitrephination was used to help locate the ostia of the frontal sinus in 1 case.The surgical effect was evaluated by following up the patients for 6-24（mean 12.7） months.ResultsThe symptoms vanished in all the cases without any recurrence.Endoscopy showed adequate opening of the frontal sinus ostia in 12 of the cases,and failed to confirm its patency in the other 3.All the patients were free of subjective symptoms.ConclusionEndoscopic frontal sinus surgery via the route anterior to the ethmoid bulla is a desirable approach to the management of isolated frontal sinusitis.
