摘要
目的 分析小儿纵隔肿瘤和囊肿的CT表现并探讨CT诊断的价值。方法 回顾性总结 5 8例经手术、病理证实的小儿纵隔肿瘤和囊肿的CT表现 ,包括神经源性肿瘤 2 5例 ,恶性淋巴瘤 15例 ,肠源性囊肿 6例 ,胸腺瘤 3例 ,畸胎瘤、脂肪瘤、支气管囊肿各 2例。囊性淋巴管瘤、淋巴血管混合瘤、胸内甲状腺瘤各 1例。其中恶性 3 4例 ,占 5 8 6%。结果 本组 5 8例CT诊断正确 5 4例 ,占 93 % ;恶性组占 97% ;良性组占 87 5 %。CT检查对小儿纵隔肿瘤和囊肿的定位及良、恶定性有很高的准确率。并能很好地与正常小儿胸腺、郎格罕细胞组织细胞增生症胸腺浸润及纵隔肺门淋巴结结核进行鉴别。结论 CT检查可清楚显示小儿纵隔肿瘤和囊肿的大小、形态、密度、范围 ,以及与纵隔大血管、气管、食管的关系。诊断小儿纵隔肿瘤和囊肿行CT检查是最有效的方法之一。
Objective ToanalyzethecharacteristicsignsandvalueofCTinthediagnosisofmediastinal tumorsandcystsinchildren Methods 5 8casesofmediastinaltumorsandcystswereexaminedbyCTscan ,all caseswereconfirmedbysurgicalandpathologicalfindings。Thisgroupincluded 2 5casesofneoplasmsofneural origin ,15casesoflymphoma( 4casesofHodgkin’sand 11casesofnon Hodgkin’s) ,6casesofgastroentericcysts , 3casesofthymictumors ;teratomas ,lipomas,bronchogeniccystshad 2casesforeach ;cysticlymphangioma , cysticmixtureoflymphangiomaandangioma ,intrathoracicthyroidadenomahad 1caseforeach。Results 5 4 casesofthisgroupwerecorrectlydiagnosedbyCTscanbeforesurgery ( 93 % ) .CTscancouldlocatethetumorof tumorsandcystsdiscriminatebetweenmalignantandbenigntumorsanddistinguishthetumorsandcystsfrom physiologicallyenlargedthymus ,pathyologicallyenlargedthymuscausedbyLangerhanscellhistiocystosis (LCH) andTBlymphadenitis Conclusions CTscancouldclearlydisplaythesize ,shape ,density ,siteandextentof tumorsandcysts Therelationbetweentumorsorcysts,vessels,tracheaandesophagusinmediastinumcouldbe clarifiedbyCTscan CTscanhasbecomethemostimportanttoolfortheinvestigationoftumorsandcystsin mediastinum
