摘要
介绍了出院计划的定义、内容、组织实施、评价,以及我国目前的应用状况及存在的问题,对出院计划在我国的推广与应用具有一定的借鉴意义。
It introduced the definition,content,organization and implementation,and evaluation of discharge planning,and current application status and problems in China.It has a certain reference significance in the promotion and application of discharge planning in China.
