急性心肌梗死患者血管内皮细胞损伤标志物的检测 被引量:8

Detection of plasma soluble markers and nuclear DNA of circulating endothelial cell in patients with acute myocardial infarction

摘要 目的:观察急性心肌梗死(Acute Myocardial Infarction,AMI)患者循环内皮细胞(CEC)核DNA改变的特点,检测血浆中血管性血友病因子(Von Willebrand factor,v WF)、血管内皮细胞蛋白C受体(Vascular Endothelial Cell Protein C Receptor,EPCR)的含量变化,探讨其临床意义。方法:应用单细胞凝胶电泳技术(SCGE)检测70例AMI组患者和70例健康对照组CEC核DNA的损伤;以酶联免疫吸附实验法(ELISA)进一步检测两组v WF、EPCR的含量。结果:SCGE显示:AMI组患者(92.92±6.95)%CEC核DNA电泳呈明显的"彗星"状,其彗尾DNA%含量为(41.12±3.77)%;对照组(10.49±4.45)%CEC核DNA表现为"彗星"状,其彗尾DNA%含量为(7.33±2.53)%。AMI组与对照组彗星率、彗尾DNA%含量相比均有显著性差异(P<0.001)。AMI组v WF、EPCR的含量显著高于对照组(P<0.01)。结论:AMI组患者血管内皮细胞明显损伤;v WF、EPCR的含量明显升高,早期检测血浆v WF、EPCR的含量可能有利于AMI的治疗,甚至预防。 Objective: To observe alteration in nuclear DNA of circulating endothelial cells,and to detect changes in concentration of plasma Von Willebrand factor(vWF) and vascular endothelial cell protein C receptor(EPCR) in acute-phase patients with acute myocardial infarction(AMI) in order to explore their clinical significance.Methods: Seventy acute-phase patients with acute AMI and 70 healthy volunteers were recruited in this study.Damages to nuclear DNA of circulating endothelial cells was assayed by single cell gel electrophoresis(SCGE) ; and concentration of vWF and EPCR were detected by ELISA. Resuits. The nuclear DNA of acute AMI patients was demonstrated like a typical comet while majority of nu- clear I)NA were demonstrated as a circle in healthy volunteers. The acute AMI group showed higher per- centage of comet cells than the control volunteers (92.92%±6.95~ vs 10.49%±4.45%, respectively, P〈0. 001), and DNA content in the comet tail was significantly increased in AMI patients as well (41.12%±-4-3.77% and 7. 33%±2.53%, respectively, P〈0. 001 ). Concentration of vWF and EPCR were also significantly higher in patients comparing with that in healthy volunteers (P〈0.01). Conclusions. Vascular endothelial cell was significant damaged; the contents of vWF and EPCR were significantly increased in pa- tients with AMI, thus detection of plasma vWF,EPCR levels may contribute to early treatment or even prevention of the onset of AMI.
