摘要
目的观察奈达铂（nedaplatin,NDP）和替加氟（tegafur,FT-207）联合化疗方案在局部晚期鼻咽癌诱导化疗中的近期疗效和毒副反应。方法采用回顾性分析方法,收集我科60例初治的局部晚期鼻咽癌病例,归为改良PF方案组和常规PF方案组,每组30例,分别接受NDP＋FT-207方案（NDP 30 mg/m2,d1-3;FT-207 600 mg/m2,d1-5;CF0.2 g,d1-5,每3周1次）和常规DDP＋5-FU方案（DDP30 mg/m2,d1-3;5-FU500 mg/m2,d1-5;CF0.2 g,d1-5,每3周1次）诱导化疗,2周期化疗结束后行鼻咽部及颈部淋巴结常规放疗,观察各组的疗效和毒副反应,并进行统计学分析。结果改良PF方案组完全缓解率（complete remission rate,CR）为83.3%,部分缓解率（partial remission rate,PR）为13.3%;常规PF方案组CR率为70%,PR率为30%,两组比较差异无统计学意义（P〉0.05）。改良PF方案组Ⅲ-Ⅳ度粒细胞减少发生率为6.6%,常规PF方案组为3.3%,两组比较差异无统计学意义（P〉0.05）。改良PF方案组Ⅲ-Ⅳ度恶心、呕吐发生率为6.7%,而常规PF方案组为26.7%,两组比较差异有统计学意义（P〈0.05）。结论奈达铂联合替加氟治疗局部晚期鼻咽癌有较好疗效,毒副反应较轻,值得在鼻咽癌诱导化疗中推广运用。
Objective To evaluate the efficacy and toxicity of induction chemotherapy involving nedaplatin（NDP） and tegafur（FT-207） in treatment of locally advanced nasopharyngeal carcinoma（NPC）.Methods Sixty patients,who were newly diagnosed and pathologically proven as NPC at III and IVa stages and admitted in our department from May 2003 to July 2010,were assigned to 2 groups（n=30）.All patient characteristics of the 2 groups were well balanced.They underwent an induction chemotherapy with NDP combined with FT-207（NDP 30 mg/m2,d1-3;FT-207 600 mg/m2,d1-5;CF 0.2 g,d1-5） or DDP combined with 5-FU（DDP 30 mg/m2,d1-3;5-FU 500 mg/m2,d1-5;CF 0.2 g,d1-5） for 21 d as one cycle respectively for 2 cycles.The clinical efficacy and toxicity were evaluated after the induction chemotherapy and radiotherapy.Meanwhile the comparative study was made between the 2 groups.Results All of cases were eligible for clinical response evaluation.Similar clinical complete response rate and partial response rate were observed between the 2 groups（83.3% vs 70.0%,and 13.3% vs 30.0% respectively,P0.05）.There was no significant difference in incidence of grade III and IV toxicity including granulocytopenia in the observer group and the control group（6.6% vs 3.3%,P0.05）.However significant difference was found in incidence of vomiting between the 2 groups（6.7% vs 26.7%,P0.05）.Conclusion Our study indicates that induction chemotherapy with nedaplatin and tegafur is a safe and effective regimen for patients with locally advanced nasopharyngeal carcinoma.
