甲亢^(131)I治疗前后前臂骨密度及相关生化指标变化分析 被引量:6

Analysis of bone mineral density and associated biochemical parameters in 102 hyperthyroid patients before and after ^(131)I treatment

摘要 目的探讨131I治疗甲状腺功能亢进(简称甲亢)前后对前臂骨密度及骨代谢相关生化指标的影响。方法 102例甲亢患者治疗前、治疗后3及6个月分别检测其血清TT3、TT4、FT3、FT4、TSH以及骨钙素(BGP)、血清碱性磷酸酶(ALP)、血清钙(Ca)和无机磷(P)的含量,治疗前后均采用DTX-200型双能X线骨密度仪检测其前臂骨密度(BMD)值。结果 1、甲亢131 I治疗后痊愈组、好转组和甲低组3个月至6个月前臂骨密度值呈上升趋势,治疗后6个月与治疗前的BMD值比较差异有显著性(P<0.05);甲亢治疗无效组BMD与治疗前BMD比较无明显差异(P>0.05)。2、甲亢131 I治疗前与治疗后6个月BGP、ALP、血清Ca比较差异有显著性(P<0.05);甲亢治疗无效组与治疗前比较无明显差异(P>0.05)。结论甲亢患者经131I治疗后前臂骨密度及其骨代谢指标均有较好恢复。 Objective To study the impact of ^131I treatment for hyperthyroidism on bone mineral density of the forearm and bone metabolism-associated biochemical parameters. Methods Serum levels of TT3, TT4, FT3, FT4, TSH, bone Gla protein ( BGP), alkaline phosphatase ( ALP), calcium (Ca) , and inorganic phosphorus (P) were measured before, and 3- and 6-month after the treatment, in 102 hyperthyroid patients. Bone mineral density (BMD) of the forearm was detected using dual-energy X-ray absorptiometry before and after the treatment. Results ( 1 ) BMDs of the forearm in the treatment groups ( cured, improved, and hypothyroid group) were increased in 3-6 months after the ^131I treatment. BMDs were significantly different between the patients before and after 6-month treatment ( P 〈 0.05). There were no significant difference in BMDs between the patients before and after 6-month treatment in the ineffective hyperthyroid treatment groups ( P 〉 0.05 ). (2) There were significant difference in the serum levels of BGP, ALP, and Ca between the hypothyroid patients before and after 6-month ^131I treatment (P 〈 0. 05 ). There were no significant difference of above parameters between the patients before and after treatment in ineffective treatment groups (P 〉 0. 05 ). Conclusion The forearm BMD and bone metabolism-associated biochemical parameters recovered after the ^131I treatment in patients with hyperthyroidism.
