丙氨酰-谷氨酰胺在重度颅脑损伤治疗中的应用价值研究 被引量:22

摘要 目的探讨丙氨酰-谷氨酰胺强化治疗对重度颅脑损伤患者的肠黏膜屏障及免疫系统功能的影响。方法将58例重度颅脑损伤患者随机分为丙氨酰-谷氨酰胺强化治疗组(实验组)和常规营养治疗组(对照组),比较两组患者治疗前及治疗后第14天的营养、免疫指标和血D-乳酸水平。结果治疗后第14天,实验组患者的营养代谢指标(血转铁蛋白、前清蛋白)、免疫指标(IgA、IgG、IgM)和D-乳酸水平与对照组比较,差异均有统计学意义(P<0.05)。结论丙氨酰-谷氨酰胺在重度颅脑损伤患者的肠黏膜屏障功能的保护、促进蛋白质的合成、改善营养状况、提高机体免疫力、减少感染的发生方面发挥一定的作用。 Objective To study the effect of alanyl - glutamine on intestinal mucosal barrier function and immune function in craniocerebral trauma patients. Methods 58 patients with craniocerebral trauma were randomly divided into two groups. One group were given intensive treatment with alanyl - glutamine ( experiment group), and the other were given common nutritional support (control group). Immune parameters, nutritional parameters and D- lactate levels were measured 14 days after treatment. Results The immune parameters (IgA, IgG, IgM), nutritional parameters (TF, PA) and D- lactate levels of the experiment group were significantly better than those of traditional group ( P 〈 0.05 ). Conclusion Alanyl - glutamine plays an important role in protecting intestinal mucosal barrier function, promoting synthesizing proteins, improving nutritional status, enhancing immune function and reducing infections of patients with severe craniocerebral trauma.
