摘要
目的探讨MR扩散加权成像结合动态增强在评价乳腺良恶性病变中的应用价值。方法对46例乳腺病患者的52处病灶同时行MR扩散加权成像(b值=800mm2/s)和动态增强MRI扫描。分别总结依据ADC值和时间-信号强度曲线评价乳腺良、恶性病变的统计学意义。统计综合考虑ADC值和时间-信号强度曲线类型评价乳腺良、恶性病变的敏感性、特异性。结果以1.228×10-3mm2/s作为界值评价乳腺病变的性质,具有明显的统计学意义(P<0.05),敏感性为87.5%,特异性为80%;以Ⅰ型曲线诊断乳腺良性病变,以Ⅱ、Ⅲ型曲线诊断乳腺癌,敏感性为90.6%,特异性为80%。如结合ADC值与时间-强化曲线对病灶良恶性分析,敏感性为93.8%,特异性为90%。结论应用DWI和时间-信号强度曲线对乳腺病变的良恶性进行评价具有重要价值。
Objective To investigate the clinical values to differentiate benign from malignant breast lesions by both MR Diffusion weighted imaging(DWI) and dynamic contrast-enhanced MR imaging.Methods All 46 patients with 52 breast lesions had been examined by both DWI(b = 800 s/mm2) and dynamic contrast-enhanced MR imaging.The statistic values of ADC and T-SI curve were studied in diagnosing breast lesions.The sensitivity and specificity of diagnosing breast lesions by both DW I and T-SI curve were also studied.Results It was statistically valuable to differentiate benign from malignant breast lesions by the values of ADC(P 0.05).If taking both the values of ADC 1.228 *10-3 mm2/s,sensitivity was 87.5% specificity 80%;if the type of I,Ⅲ T-SI curve together as the only standard to appraising breast carcinoma,sensitivity was 90.6% specificit y80%.If taking both the values of ADC and T-SI curve together to apraising breastmalignant lesion,sensitivity was 93.8% specificity 90%.Conclusion It is helpful to differentiate breast malignantand benign lesions by combining DWI and T-SI curve.
