摘要
目的 :利用大鼠小肠缺血再灌流 (IIR)模型 ,评价肿瘤坏死因子 (TNF)在IIR所致肺损伤发病过程中的作用 ;观察大黄对TNF影响 ,探讨大黄防治肠源性肺损伤的机理。方法 :SD大鼠随机分为 4组 :1)肠缺血再灌流组 (n =2 4) ;2 )假手术组 (n =16 ) ;3)大黄治疗组(n =2 4) ;4)安慰剂组 (n =16 )。以1 2 5 I标记小牛血清白蛋白 (BSA)肺摄取指数作为评价肺毛细血管通透性的指标 ,以髓过氧化物酶 (MPO)作为评价多聚核白细胞 (PMN)在组织中聚集的指标 ,采用放射免疫分析法 ,分别测定各组动物不同时间血浆及肺组织TNF含量。结果 :大黄可明显抑制再灌流导致的肺MPO活性升高 (P <0 0 1)及肠缺血期和再灌流早期出现的血浆及肺组织TNF水平升高 (P <0 0 1) ;降低肺毛细血管通透性 (P <0 0 1)。结论 :早期应用大黄有助于防止大鼠IIR所致肺损伤的发生 ,这种作用至少部分是通过抑制IIR诱导的TNF释放而实现的。
Aim:Using a rat intestinal ischemia and reperfusion(IIR)model to evaluate the possible role of tumor necrosis factor(TNF)in the development of lung injury after intestinal ischemia reperfusion(IIR),and the effect of rhubarb on TNF and this kind of lung injury.Methods:Adult Sprague Dawley rats were divided into four groups:1)ischemia and reperfusion alone(n=24);2)those animals receiving rhubarb 600mg/kg 30 minutes before reperfusion(n=24);3)those animals receiving NS instead of rhubarb,serving as placebo(n=16);4)the remaining 16 animals were shamly operated(laparotomy),serving as controls.Lung permeability was measured using bovine serum albumin(BSA)labeled with 125 I.The concentration of plasma and lung TNF in different groups and in different time were measured by radioimmunoassay,and the activity of lung myeloperoxidase(MPO)which served as an index of polymorphonuclear(PMN)was measured by chromometry.Results:Rhubarb down regulated the activity of MPO in the development of lung injury after IIR(P<0 01),decreased the high level of TNF during ischemia and early stage of reperfusion,and attenuated the increase of the pulmonary microvascular permeability significantly(P<0 01).Conclusion:The result of this study show that using rhubarb in early stage may contribute to prevent gut origin lung injury in rats,this protective effect may at least partly due to the inhibition of TNF.
