摘要
目的探讨甘精胰岛素联合瑞格列奈治疗对新诊断老年2型糖尿病的降糖疗效、安全性、对胰岛B细胞功能及胰岛素抵抗的影响。方法新诊断老年2型糖尿病63例，随机分为甘精胰岛素联合瑞格列奈组及预混人胰岛素组，治疗两周，比较治疗前后血糖（PG）、空腹胰岛素（Fins）、空腹C肽（FC—P）、胰岛β细胞分泌指数（HOMA—β）及胰岛素抵抗指数（HOMA—IR）的变化，比较两组治疗血糖达标时间、胰岛素用量、低血糖发生频率及7点血糖水平的标准差（SDBG）。结果治疗后两组患者PG、HOMA—IR均明显下降（P〈0．05）、Fins、FC—P、HOMA—islet均明显升高（P〈0．05），两组间上述指标无统计学意义（P〈0．05）；甘精胰岛素组胰岛素用量、低血糖发生频率及SDBG均低于预混人胰岛素组（P〈0．05），但血糖达标时间长于预混人胰岛素组（P〈0．05）。结论甘精胰岛素联合瑞格列奈能有效、平稳、安全降糖，并可改善新诊断老年2型糖尿病患者胰岛β细胞功能及胰岛索抵抗。
Objective To observe the effects of insulin glargin combined with Repaglinide on antihyperglycemic effects, security, β - cell function and insulin resistance in newly diagnosed senile diabetes. Methods 63 newly diagnosed senile diabetes eases were randomly assigned to insulin glargin combined with Repaglinide group and premixed human insulin group for two weeks. Plasma levels of glucose（ PG）, insulin, C - peptide, and the islet β - cell function （ HOMA - islet） and insulin resistance （ HOMA - IR） were measured before and after treatment. The blood glucose targeting period, dosage of insulin, incidence of hypoglycaemia and standard deviation of blood glucose（SDBG） were compared between two groups. Results There were significant decreases in PG and HOMA - IR（ P 〈 0.05 ）. Fins, FC - P and HOMA - β were significant increased （ P 〈 0.05）. Insulin glargin group had a better effect of insulin dosage, incidence of hypoglycaemia and SDBG, but spended longer time to achieve better standards of blood glucose （ P 〈 0.05 ）. Conclusion For newly diagnosed senile diabetes, the treatment of insulin glargin combined with Repaglinide is effective, stable and safe, which can evidently improve β - cell function and relieve IR.
