摘要
目的研究逆行胰胆管造影术联合自拟化石利胆汤治疗胆总管结石的临床疗效。方法对200例胆总管结石患者行逆行胰胆管造影术(ERCP)、十二指肠乳头切开术(EST)、网篮取石术、鼻胆管引流术(EBND),术后随机分为2组,中药组(A组)100例:在常规抗感染、抑酸、抑酶和补液治疗基础上加用自拟化石利胆汤;空白对照组(B组)100例:只给予常规抗感染、抑酸、抑酶和补液治疗。监测术后第1、7天患者总胆红素、淀粉酶、白细胞总数和症状体征评分。结果 A组总胆红素、淀粉酶、白细胞总数下降幅度均显著高于B组,P<0.05;A、B组症状体征评分的下降幅度比较中,体温积分2组比较无显著差异,A组的腹痛、腹胀、排便积分显著低于B组,P<0.05。结论 ERCP+EST+ENBD联合化石利胆汤不但可以有效治疗胆总管结石,而且化石利胆汤可以减少胆管感染及术后胰腺炎发生,使肝功恢复更快,并使患者相关症状明显减轻。
【Objective】To study the treatment effect of cholelithiasis with ERCP Huashilidan Decoction made by us. 【Methods】Two hundreds of patients undergoing ERCP, EST, ENBD for choledocholithiasis were randomLy divided into two groups, with 100 patients in each group. The control group B was given the normal antiinflammatory, acidity inhibitors, inhibitory activity and rehydration therapy. The traditional Chinese medicine group A was given the treatments of Huashilidan Decoction on the basis of the normal therapy. They were monitored the total of bilirubin, amylase and WBC at the first and the seventh day after operation. The hepatobiliary and pancreatic ab-dominal ultrasound spleen and symptom-sign score were carried out. 【Results】The rate of descent of bilirubin, amylase and WBC of the group A was larger than that of the group B, P 0.05. To compare the descender about symptom scores of A B, it was found that there was no significant difference in temperature points, but abdominal pain, abdominal distension and defecation in the group A was significantly lower than group B, P 0.05.【Conclusions】ERCP, EST, ENBD Huashilidan Decoction may effectively treat cholelithiasis, and Huashilidan Decoction can reduce the occurrence of biliary tract infection and postoperative pancreatitis.
