摘要
目的探讨血管内皮生长因子(VEGF)、肿瘤坏死因子(TNF-α)在糖尿病肾病(DN)患者不同阶段的表达水平及相关性。方法用ELISA法测定81例糖尿病肾病及20例健康者对照组血清中VEGF和TNF-α水平,并进行分析。结果 (1)VEGF水平在糖尿病(DM)组、微量蛋白尿(MA)组、临床蛋白尿(ODN)组明显高于健康对照组,差异有统计学意义(均为P<0.01);血清中TNF-α水平在DM、MA、ODN 3组明显高于健康对照组,差异有统计学意义(均为P<0.01)。(2)在糖尿病组患者,血清中VEGF与TNF-α浓度呈正相关(r=0.712,P<0.01);血清VEGF和TNF-α浓度与UAER的升高呈正相关(r1=0.801,P<0.01;r2=0.732,P<0.01)。(3)糖尿病患者血清中VEGF水平、TNF-α浓度与低密度脂蛋白(LDL-C)水平、糖化血红蛋白(HbA1C)呈正相关(r3=0.345,P<0.05,r4=0.455,P<0.05)。结论血清中VEGF和TNF-α水平在不同时期DN患者中有不同程度的增高,它们与UAER也呈正相关关系,有可能成为监测DN病情变化的有效指标之一。
Objective To explore the significance of vascular endothelia growth factor（VEGF）and tumor necrosis factor-α（TNF-α） in the peripheral blood of the patients with diabetes nephropathy（DN）.Methods VEGF and TNF-α were measured in eighty-one patients with DN and twenty control subjects（control group） by enzyme-linked immune absorption assay（ELISA）,and the correlation was analyzed.Results（1）VEGF level was significantly higher in the patients of DM,MA and ODN groups than in the control group;TNF-α level was significantly higher in the patients of DM,MA and ODN than in the control group.（2）The levels of VEGF in peripheral blood were positively correlated with the levels of TNF-α;the levels of VEGF and TNF-α in peripheral blood were correlated with UAER.（3）The levels of VEGF and TNF-α in peripheral blood were positively correlated with the levels of low density lipoprotein cholesterol（LDL-C） and HbA1C.Conclusion The levels of VEGF and TNF-α has the varying degree of increase in the different stage DN patients,and show the positive relation with UAER,which is possible to become the effective index to monitor the condition change of DN.
