摘要
目的提高结核病和艾滋病双重感染患者发现率、治疗成功率和减少患者死亡。方法省、市、县三级分别建立结核病和艾滋病防治机构合作机制;对结防机构新登记的肺病人进行HIV抗体检测;对艾防机构辖区内可随访的HIV/AIDS患者进行TB检测;结核病人治疗按《中国结核病防治规划实施工作指南》实施。结果 2007年10月-2009年10月结防机构新登记肺结核患者6 590例,进行HIV检测6 035例(91.6%),HIV阳性31例(0.5%);同期艾防机构累计完成HIV/AIDS患者TB筛查2 139人次,查出结核病94例(4.4%),其中,肺结核为87.2%,肺外结核为12.8%;2007年10月-2008年10月共发现TB/HIV患者72例,对其中66例进行了抗结核治疗,治愈13例,完成疗程28例,治疗成功率62.1%,死亡14例(21.2%),丢失6例(9.1%)。结论 1、结防机构和艾防机构合作实施双向筛查,是提高TB/HIV双重感染患者发现率的切实有效的方法。2、加强治疗管理、开展关爱活动、争取患者配合是提高治疗成功率、减少死亡有效措施。
Objective To improve the detection rate and the success treatment rate of TB/HIV dual infection patients,and to reduce the patient＇s death.Methods The cooperation mechanisms of TB/AIDS control prevention agencies in the levels of province,cities and counties were established.HIV antibody detection was conducted among the newly diagnosed tuberculosis patients.Tuberculosis detection was conducted in the diagnosed HIV/AIDS patients who could be followed up in the areas of AIDS control prevention agencies＇ jurisdiction.Tuberculosis patients were treated according to ＂China Tuberculosis Control and Prevention Plan： Guideline for Programme Implementation＂.Results There were 6,590 newly diagnosed tuberculosis patients in TB control prevention agencies from October,2007 to October,2009.HIV antibodies were detected in 6,035（91.6%） patients,and 31（0.5%） patients were HIV positive.At the same time,TB detection was conducted among 2,139 HIV/AIDS patients in AIDS control prevention agencies,and 94（4.4%）patients were diagnosed with TB,including pulmonary tuberculosis（87.2%） and extra pulmonary tuberculosis（12.8%）.There were 72 HIV-infected tuberculosis patients diagnosed from October 2007 through October 2008.Among them,66 TB/HIV patients were received anti-tuberculosis treatment,and 13 patients were cured.28 TB/HIV patients had completed the treatment,with the success treatment rate of 62.1%.14（21.2%） TB/HIV patients died,and 6（9.1%） TB/HIV patients were lost.Conclusions Implementing TB/HIV double screening through the corporation of TB and AIDS control prevention agencies is the effective way to improve the detection rate of TB/HIV dual infection patients.Strengthening treatment and management,developing care activities,and trying to gain the patients＇ cooperation are the effective measures in improving the success rate of treatment and reducing the patient＇s death.
