摘要
目的：探讨乳腺癌在MSCT俯卧位三维重建下的CT表现,评价其诊断价值.方法：对38例CT诊断为乳腺癌的病人进行CT表现与手术病理结果：进行分析比较,其中37例经病理证实,另1例经临床随访证实.结果：（1）CT诊断率38例乳腺癌CT诊断正确37例,诊断率为97.3%,1例随诊.（2）乳腺癌的主要CT特征为：①肿块边缘不规整,有分叶和毛刺;②强化后CT值平均升高44Hu以上,无包膜;③肿块内有沙砾状钙化;④腋窝淋巴结境界欠光整,可有融合,呈＂串珠状＂改变;⑤肿块侵犯胸壁等.结论：CT是鉴别乳腺良恶性肿块的一种重要方法,并能发现临床扪诊阴性的乳腺病变,为乳腺癌术前分期及手术方式选择具有重要价值.
Objective：Discusses breast cancer to bend down under the decubitus three dimensional reconstruction in MSCT the CT performance, appraises its diagnosis value. Methods：Carries on the CT performance and the surgery pathology to 38 example CT diagnosis for breast cancer patientthe Results：Carries on the analysis comparison, 37 examples confirmed after the pathology,another 1 example after clinical revisit confirmation. Results：（1） CT diagnosis rate 38 example breast cancer CT diagnoses the correct 37 examples, the diagnosis rate is 97.3 %, 1 examples along with examines. （2） breast cancer main CT characteristic is：①The tumour edge is not neat,influentials the leaf and the burr; ②After the strengthening the CT value elevates above equally 44Hu, does not have a package of membrane;③In lhe tumour has the sand psephitic calcification; ④The armpit lymph node boundary owes the light entire,may have the fusion,assumes ＂the string beaded＂ the change;⑤Tumour infringement thoracic wall and so on. Conclusion：CT is distinguishes the mammary gland good malignant turnout one important method,and can discover the clinical palpation negative mammary gland pathological change,by stages and the surgery way choice has thc important value for breast cancer technique in front of.
