摘要
目的调查门诊慢性前列腺炎/慢性骨盆疼痛综合征(CP/CPPS)患者的特征及症状的严重程度。方法对经临床确诊,满足CP/CPPS标准的612例患者进行了问卷调查。内容包括:①年龄、身高、体重、民族、职业、文化程度、经济状况等;②疾病既往史、就诊次数、病程、前列腺液检查、性生活史以及治疗情况;③美国国立慢性前列腺炎症状指数(NIH-CPSI);④根据患者对NIH-CPSI中疼痛或不适及排尿症状的报告,评价症状的严重程度。结果①<40岁的患者占70.59%,并且随年龄的增加患者的数目减少;②患者的病程、民族及生活状况与CP/CPPS无关,然而年龄、文化程度、家庭居住地及家庭经济状况等均与CP/CPPS有关;③52.28%的患者有腰部以下耻骨或膀胱区疼痛或不适,23.03%的患者有尿痛,性功能障碍者占21.90%;④尿频者占65.85%,74.51%的患者有排尿不适。结论随着年龄的增加,患CP/CPPS的数目相对减少。主要的症状是排尿功能障碍,如尿频和排尿不适。CP/CPPS患者临床症状的严重程度与文化程度、居住地及经济状况直接相关,疾病久治不愈可以使患者的临床症状加重。
【Objective】To investigate and analyze characteristics and symptom severity of chronic prostatitis/chronic pelvic pain syndrome in urological outpatients.【Methods】Questionnaire surveys were completed by 612 subjects who met the criteria of chronic prostatitis after clinical diagnosis.All patients were selected from 3 urological clinics of Jinzhou city,using a structured interviewer-administered questionnaire that included information on：① age,height,weight,nation,occupation,economic status,and educational background;② personal medical history,times of clinic visiting,course of disease,expressed prostatic secretions（EPS）,sexual activity,and treatment status;③score of the National Institute of Health-Chronic Prostatitis Symptom Index（NIH-CPSI）;and ④symptom severity was assessed with the NIH-CPSI according to patients self-report about pain and discomfort,and urination.【Results】①Among the 612 subjects selected,70.59% of the patients were aged 40 years and patients decreased with increasing age.②Symptom severity had a positive correlation with age,educational background,and economic status,and had no correlation with course of disease,nation and living status,on the contrary.③ Discomfort and pain in pubic or bladder area below waist was 52.28%,pain on urination was 23.03%,sexual discomfort was 21.90%.④ Urinary frequency was 65.85%,and voiding discomfort was 74.51%.【Conclusion】There are fewer patients with chronic prostatitis as age increases.The main symptoms are voiding dysfunction,e.g.frequent urination or discomfort.Symptom severity in patients with chronic prostatitis has direct relationship with educational background,habitation and economic status;however prolonged therapeutic process aggravates the severity of symptom.
