实时三平面组织多普勒成像技术评价糖尿病患者左心室功能及与HbA1c的关系 被引量:5

Assessment of left ventricular function in diabetes mellitus patients with real-time triplane tissue Doppler imaging and the relationship with hemoglobin A1c

摘要 目的应用实时三平面组织多普勒(Triplane-TDI)技术评价糖尿病(DM)患者左心室功能及其与糖化血红蛋白(HbA1c)的关系。方法 50例DM患者(DM组)和50名健康志愿者(正常对照组)接受组织多普勒成像(TDI)。在三平面成像基础上,应用定量组织速度、应变率技术测量左心室各壁二尖瓣环运动速度、不同节段(基底段、中间段)的运动速度及应变率;检测2型DM患者的HbA1c。结果与正常对照组相比,DM组左心室整体收缩功能减低,但差异无统计学意义(P>0.05),整体舒张功能及局部收缩、舒张功能减低,差异有统计学意义(P<0.05);HbA1c与舒张功能参数呈线性相关。结论 Triplane-TDI技术能准确评价DM患者早期心肌舒缩功能的变化;参考HbA1c水平可了解DM患者心脏舒张功能的受损情况。 Objective To discuss the clinical value of real-time triplane tissue Doppler imaging (Triplane-TDI) in evaluating left ventricular function in diabetes mellitus patients, and to evaluate the relationship between cardiac function change and the level of hemoglobin A1c. Methods Fifty DM2 patients and 50 age-and sex-matched normal controls were enrolled. Peak velocity of the mitral annulus, peak velocity and peak strain rates of the basal segment and middle segment were measured with real-time triplane quantitative tissue velocity imaging and strain rate imaging. HbA1c of DM2 patients were obtained. Results There was no significant difference in left ventricular global systolic function between the two groups (P0.05). Compared with control group, left ventricular global diastolic and regional systolic and diastolic function reduced in DM2 (P0.05). Left ventricular diastolic dysfunction was linear related with HbA1c. Conclusion Triplane-TDI can be used as a sensitive mean for prediction of the subclinical cardiac dysfunction in DM2 patients. According to HbA1c, impaired diastolic function in DM2 patients can be understood.
