摘要
目的 对硬膜外术后镇痛用药是否对脊髓有神经毒性进行研究,以指导临床用药。方法 健康家兔48 只,硬膜外穿刺成功后,随机分为6 组,每组8 只,生理盐水(1m l)对照组(S组),布比卡因(1m g/kg)组(B组),芬太尼(2μg/kg)组(F组),曲马多(1.2m g/kg)组(T 组),氟哌利多(0.05m g/kg)组(D组),咪达唑仑(0.1m g/kg)组(M 组)。6 组均给药2次/d,间隔12小时,连续给药3天,继续观察至第7天处死兔作光镜和电镜检查。结果 电镜下各组均未见坏死性改变。光镜下D组有2 只,M 组有1 只兔的脊髓病变较严重,脊髓灰质坏死,结构破坏,神经细胞减少。其余组同S组。结论 常用浓度的布比卡因、芬太尼、曲马多注入硬膜外腔安全、可靠。氟哌利多和咪达唑仑注入兔硬膜外腔见少部分动物的脊髓有严重破坏性病变,建议临床慎用。
Objective To determine the pathologic effects of 5 kinds of widespreadly used drugs (bupivacaine,fentanyl,droperidol,tramadol,midazolam) on spine cord Methods After successive 3 day epidural administration of the drugs and a 3 day observation,the rabbits were killed and the spinal cord was examined under optical and electron microscope Results Serious damages of neuron were found in 2 animal from group droperidol and 1 from group midazolam under optical microscope Some reversible changes were found in all groups under electron microscope Conclusion Epidural administration of clinical dosage of bupivacaine,fentanyl,or tramadol is safe The long term neurotoxicity of droperidol and midazolam may occur
