摘要
目的探讨老年住院患者难免压疮的护理方法 。方法 用Norton评分法评估,筛选出老年高危压疮患者331例,运用循证护理方法 和步骤,提出问题,查找应用证据,并根据患者不同的个体状况,制定压疮预防和治疗的护理措施,实施个体化的护理。结果 331例患者中,难免性压疮发生率为17.52%,其中Ⅰ度压疮发生率为14.20%,Ⅱ度压疮发生率为3.32%。压疮患者的创面经个体化的治疗和护理后,329例愈合(愈合率99.4%)。结论应用循证护理,高危老年患者的难免性压疮能有效得到预防和治疗。
Objective To explore the best nursing care of elderly patients with inevitable pressure sore.Methods A total of 331 elderly high-risk patients with pressure sore was evaluated using the assessment table of Norton.The problems and evidences were collected according to the method and process of evidence-based nursing and the individualized nursing plan was established and practiced.Results Of 331 patients,the incidence rate of pressure sore was 17.52% （14.20% in the patients with gradeⅠand 3.32% in the patients with gradeⅡ）.The pressure sores were cured in 329 cases with a healing rate of 99.4%.Conclusion The inevitable pressure sore can be prevented and cured effectively in elderly patients at high-risk as long as the evidence-based nursing is taken properly.
